Psoriazis vulgaris tanılı hastalarda oral bulgular ile ağız ve diş sağlığının değerlendirilmesi

Özet
Amaç: Psoriazis vulgaris tanılı hastalarda oral bulgu varlığı ve bu bulguların hastalığın klinik özellikleriyle ilişkisini araştırmak ve hastaların ağız ve diş sağlığını değerlendirmektir. Yöntemler: Psoriazis vulgaris tanılı 85 hasta ile kontrol grubunu oluşturmak üzere 100 birey çalışmaya dahil edildi. Tüm bireylerde oral bulgu, obezite varlığı, çürük, kayıp ve dolgulu diş (decayed, missing, filled, teeth) (DMFT) değerleri ile ağız ve diş sağlığı değerlendirildi. Bulgular: Fissürlü dil ve coğrafik dil sıklığı hasta grubunda anlamlı oranda yüksekti (sırasıyla, p<0,001; p=0,049). Psoriazis vulgaris tanılı hastalar içinde, sistemik immünosüpresif tedavi alanlarda almayanlara kıyasla fissürlü dil sıklığı anlamlı oranda yüksekti (p=0,002). DMFT ortalaması hasta ve kontrol grubunda benzerdi (p>0,05). Hasta grubu klinik özelliklerine göre değerlendirildiğinde, geç başlangıçlı psoriazis hastalarında erken başlangıçlılara kıyasla, obez psoriazis hastalarında obez olmayanlara kıyasla diş kaybı daha fazla iken; erken başlangıçlı psoriazislilerde geç başlangıçlı psoriazislilere kıyasla ve immünosüpresif tedavi almayan psoriazis hastalarında immünosüpresif tedavi alan psoriazis hastalarına kıyasla dolgulu diş sayısı daha yüksekti (sırasıyla, p=0,017; p=0,002; p=0,038; p=0,019). Sonuç: Psoriazis hastalarının ağız ve diş sağlığı açısından düzenli muayenesi ve bilgilendirilmeleri, diş sağlığının korunmasında fayda sağlayabilir.
Objective: To investigate the presence of oral findings and relationship between these findings and clinical features in patients with psoriasis vulgaris, and to evaluate the oral and dental health of the patients. Methods: This study included 85 patients with psoriasis vulgaris and 100 participants as a control group. Oral findings, presence of obesity, decayed, missing, filled, teeth (DMFT) values, and oral and dental health were evaluated in all participants. Results: The frequency of fissured tongue and geographic tongue was significantly higher in the patient group (p<0.001; p=0.049, respectively). Among patients diagnosed with psoriasis vulgaris, fissured tongue was found more frequently in patients receiving systemic immunosuppressive treatment than in patients non-receiving immunosuppressive treatment (p=0.002). The mean of the DMFT was similar in patient and control group (p>0.05). When the patient group was evaluated according to clinical characteristics of the disease, the number of missing teeth was higher in patients with late-onset psoriasis than early-onset group, and in patients with obese than nonobese group, whereas the number of filled teeth was higher in patients with earlyonset psoriasis than late-onset group, and patients non-receiving immunosuppressive treatment than patients receiving immunosuppressive treatment (p=0.017; p=0.002; p=0.038; p=0.019, respectively). Conclusion: Regular examination and informing of psoriatic patients in terms of oral and dental health may be beneficial in maintaining dental health.
