Özet
A 39-year-old man presented to our hospital complaining of fever with chills, night
sweats, and headaches of 3 months’ duration. The patient was suffering from severe
lumbago, exacerbated by movement. Although he had physical therapy for his back
pain, there was no improvement.
On examination, his pulse was 80/min, blood pressure 110/70 mmHg, and
temperature 37.8°C. Liver palpable 2 cm below the right costal margin was firm and
non-tender. Other system examinations were unremarkable.
Investigations revealed haemoglobin of 13.9 gm, ESR 60 mm/h, AST 45 U/L, and
ALT 56 U/L (normal 0–40 U/L). Remaining blood biochemical parameters were
within normal limits.
Brucella serology in blood, done by the standard tube agglutination (STA) method,
was positive with a titre of 1:320. Blood cultures were positive for Brucella spp by the
BACTEC system.
A plain lateral radiography revealed blunting of the anterosuperior corner of the L4
vertebrae (Figure 1). This finding arose the suspicion of erosion which was confirmed
with a subsequent lumbar spine CT examination (Figure 2).
