Özet
Çift pilor distal mide, antrum ve ampulla arasında kısa aksesuar bir kanal oluşması şeklindedir. Endoskopi ile rahatlıkla tanı konulabilen, kojenital veya akkiz olabilen bir patolojidir. Bu yazıda biri 75 diğeri 39 yaşında olan çift pilorlu iki olgu sunulmuştur. Birinci olguda çift pilor, üst GİS kanama sonrası yapılan endoskopide saptandı. İkinci olguda ise şiddetli mide ağrısı yakınmasının tanısı amacıyla yapılan endoskopide ant- rumda çift pilor saptandı. Her iki olguda da duodenit mevcuttu. Uzun süreli peptik ülser nedeniyle hastalarda çift pilor geliştiğini düşünüyoruz. Hastalar, proton pompa inhibitörü ile medikal tedavi uygulandı ve poliklinik takibe alındı. Çift pilorlu vakalarda öncelikle medikal tedavi uygulanmalı ancak dirençli vakalarda veya komplikasyon gelişenlerde cerrahi tedavi akılda tutulmalıdır.
Double pylorus is a short accessory canal formation between the distal stomach, antrum, and ampulla. It is a congenital or acquired pathology which can be easily diagnosed by upper gastrointestinal endoscopy. We present two cases of double pylorus; one patient was 75 and the other was 39 years old. In the first case, double pylorus was diagnosed by upper gastrointestinal endoscopy which had been performed after the occurrence of gastrointestinal bleeding. In the second case, endoscopy performed for a diagnosis of severe epigastric pain revealed an antral double pylorus. Both cases presented with duodenitis. We think that double pylorus occurred after long-term peptic ulcers in our patients. We treated our patients medically with proton pump inhibitors and outpatient follow-up. Medical treatment should be the first choice in cases of double pylorus, but complicated or resistant cases should be referred for surgery.
