Üniversite hastanesi üroloji polikliniğine başvuran hastaların geliş durumlarının değerlendirilmesi

Özet
AMAÇ: Verilen sağlık hizmetinin bireye ihtiyacı olduğu basamakta sağlanması, sağlık hizmeti
örgütlenmesinde nitelikli bir hizmet üretimi için gerekmektedir. Çalışmamızın amacı üçüncü basamak
sağlık hizmeti veren merkezimize başvuran hastaların başvuru nedenlerinin ve başvuruyu etkileyen
faktörlerin belirlenmesi ile mevcut durumun ortaya konulmasıdır.
YÖNTEM: Çalışmamız tanımlayıcı tipte planlanmış olup, Aralık 2011-Mart 2012 tarihleri arasında
üroloji polikliniğine herhangi bir nedenle başvuran katılımcılar gönüllülük esasına göre belirlenmiş ve 14
sorudan oluşan soru formu hekim tarafından doldurulmuştur. Soru formunda, polikliniğine başvuran
katılımcının yaşı, eğitim durumu, geliş şikâyeti, şikâyetlerinin başlamasından itibaren geçen süre, daha
önce başka kurumdan tedavi alma durumu ve üniversite hastanesi polikliniğini tercih etme nedenleri
sorgulandı.
BULGULAR: Çalışmaya katılan 337 kişinin %23,7’si kadın, %76,3’ü erkekti. Katılımcıların %61,7’si
şikâyetleri nedeniyle gelmeden önce tıbbi yardım almışken, % 38,3’ü daha önce herhangi bir sağlık
kurumunda görülmeden, polikliniğe doğrudan başvuruda bulunmuştu. Doğrudan üniversite hastanesi
polikliniğine başvuru, erkeklerde anlamlı derecede fazlaydı (p=0.011) (Tablo 1). Doğrudan başvurular
eğitim düzeyi arttıkça anlamlı derecede artmaktaydı. Şikâyetleri nedeniyle başka kurumda görülen
hastalardan istenen tetkikler ile doğrudan başvuruda bulunan hastalardan istenen tetkiklerin birinci
basamakta yapılabilen ya da ileri aşama tetkikler olması açısından aralarında anlamlı farklılık saptanmadı
(p=0.134).
SONUÇ: Sağlık kaynaklarının daha etkili ve sonuca yönelik kullanılabilmesi için birtakım yasal
düzenlemelerin yapılması yanısıra, sağlık hizmeti kullananlarında sağlık okuryazarlığının artırılması
açısından konuyla ilgili eğitimler verilmesi gerekmektedir.
AIM: Providing individuals with the healthcare services where needed is essential for the delivery of a quality service in the organization of healthcare services. The purpose of our study is to determine the reasons why patients apply to our center, which offers a tertiary health service, the factors which affect their application and to put forward the current situation.. METHOD: Our study, which was conceived as of the descriptive type, was carried out through a survey of 14 questions. The participants were determined among the patients who applied to the urology clinic for any reason on a voluntary basis. The 14-item questionnaire, in which the age, the educational status and the initial complaint of the participant, the time elapsed since the beginning of the complaints, whether they had received medical treatment from any other institutions before, and the reason why they preferred a university hospital were examined, was completed by the physician.. RESULTS A total of 337 participants, 23.7% of whom were female, and 76.3% male, attended. 61.7% of them had received earlier medical attention because of their complaints; 38,3% had not received any previous medical attention in any health institution and had to apply directly to the tertiary healthcare center. The number of direct applications to outpatient clinics of university hospitals was significantly higher in men (p:0.011)(table 1). As the education level got higher, the number of direct applications increased as well. No significant difference was noticed in comparison between the first and advanced level examinations of those who were checked in other medical facilities and those who applied directly (p:0,134). CONCLUSION For a more effective and result-oriented use of health resources, those who benefit from the healthcare services must also be provided with a relevant training for the purpose of increasing health literacy and certain legal arrangements must be made.
