Öğe A Case with Tracheobronchopathia Osteochondroplastica Presenting Upper Airway Obstruction(Duzce Univ, 2009) Akin, Nese; Dumlu, Talha; Balbay, Ege Gulec; Yildirim, Umran; Karatas, Naciye; Kucuk, Ebru; Annakkaya, Ali NihatTracheobronchopathia Osteochondroplastica (TO) is a rare, slowly progressive benign condition characterized by the presence of bony and cartilaginous nodules in the tracheal and bronchial submucosa. 65 years old man attending to chest disease policlinic with chest pain and upper airway obstruction in his spirometry was diagnosed as TO according to thorax CT and fiberoptic bronchoscopy findings. On computerized tomography, 140x110x50 mm in diameter solid mass was detected in upper pole of the left kidney on computerized tomography (CT). The patient underwent left radical nephrectomy and histopathological examination of the specimen was reported as pyelonephritis. Six months after the operation, the patient admitted to our clinic with huge intraabdominal mass. It was defined as hematoma or abcess on CT. One thousand cc necrotic clot like material was evacuated in exploration. Histopathological examination of this material indicated desmoplastic small round cell tumor.Öğe Lentigo maligna and contiguous pigmented lesion(Deri Zuhrevi Hastaliklar Dernegi, 2013) Erdem, Hawa; Yildirim, Umran; Aliagaoglu, Cihangir; Turan, Hakan; Uzunlar, Ali KemalLentigo maligna (LM), a subtype of melanoma in-situ, is seen mostly in patients between 50 and 80 years old and accounts for ten to fifteen percent of skin cancer cases. LM is a traditional term for atypical pigmented macular lesions. LM occurs on severely sun damaged skin and it is usually on the face of elderly patients. Histological diagnosis of atypical pigmented macular lesions have extensive range from solar lentigo to in-situ melanoma (lentigo maligna pattern) or invasive invasive melanoma (lentigo maligna melanoma). The presence of solar lentigo, pigmented actinic keratosis, or reticulated seborrheic keratosis in incisional specimen could misguide the pathologist and clinician, they might think that lentigo maligna is not present. Therefore, excisional biopsy is needed in such cases. Our aim in presenting this case was to emphasize the importance of clinicopathologic correlation and to attract attention on the lesions adjacent to lentigo maligna which can lead to a misdiagnosis.Öğe PRIMER CUTANEOUS ACTINOMYCOSIS IN DIABETIC PATIENTS(Gunes Kitabevi Ltd Sti, 2011) Alagaoglu, Cihangir; Yildirim, Umran; Albayrak, Hulya; Goksugur, Nadir; Ozcelik, DeryaActinomycosis is a chronic suppurative granulomatous infection caused by gram positive bacterium. Enfection especially affects the head, neck, thorax and abdomen. Primary cutaneous involvement is uncommon and has an association with trauma and local ischemia. We present one such case of primary cutaneous actinomycosis on the elbow of a 68 year old man with diabetes and external trauma. This patient was admitted to our clinic with a 10-12cm size blue-purple plaque. Skin biopsy revealed the characteristic 'sulphur granules' of actinomycosis in the deep dermis After the diagnosis, initially he was treated with penisilin and cotrimaxazol with a little effect. Treatment was continued with amoksisilin and cotrimaxazol and he respond to this medical treatment partially. After this medical therapy skin graft operation was performed. Lesions relapsed six months later. Medical therapy was repeated and still continues.Öğe An unusual presentation of peroneal neuropathy secondary to pigmented villonodular synovitis: a case report(Springer, 2009) Orhan, Zafer; Oktas, Birhan; Yildirim, UmranPigmented villonodular synovitis (PVS) is a benign proliferative disorder of unknown origin that affects synovial joints, most commonly the knee. The joint knee can be affected by localized or diffuse form. Diffuse PVS, the aggressive form of the disease is much more problematic, especially when it extends extra-articularly, and is associated with high recurrence rates. Although this disease is categorized as an inflammatory process rather than a neoplasm, it may be locally destructive and involve muscles, tendons, bone and skin. Neural involvement of the disease is rather rare, and only limited knowledge about neuropathy due to PVS we have yet. The presentation of the disease in our patient is a peroneal neuropathy which is the first reported case in English language literature of PVS of the knee seen with extra-articular tissue involvement.
