Öğe Brucellar spondylitis (Pedro Pons' sign)(2007) Karabay, Oğuz; Gürel, Kamil; Sirmatel, Öcal; Sırmatel, FatmaA 39-year-old man presented to our hospital complaining of fever with chills, night sweats, and headaches of 3 months’ duration. The patient was suffering from severe lumbago, exacerbated by movement. Although he had physical therapy for his back pain, there was no improvement. On examination, his pulse was 80/min, blood pressure 110/70 mmHg, and temperature 37.8°C. Liver palpable 2 cm below the right costal margin was firm and non-tender. Other system examinations were unremarkable. Investigations revealed haemoglobin of 13.9 gm, ESR 60 mm/h, AST 45 U/L, and ALT 56 U/L (normal 0–40 U/L). Remaining blood biochemical parameters were within normal limits. Brucella serology in blood, done by the standard tube agglutination (STA) method, was positive with a titre of 1:320. Blood cultures were positive for Brucella spp by the BACTEC system. A plain lateral radiography revealed blunting of the anterosuperior corner of the L4 vertebrae (Figure 1). The pathogenic organism responsible for hydatid cyst is Echinococcus granulosus. Despite the fact that the characteristic cysts of the disease usually occur in liver and lungs, they may also occur in aberrant localisations. Subjects either present with symptoms of infection itself or local complications due to compressive mass effect of the cyst. Here, we report a case of secondary hypertension due to extrinsic compression of the right renal artery by a giant hepatic hydatid cyst. A 78-year woman presented with poorly controlled hypertension. She has been treated for hypertension with an angiotensin converting enzyme inhibitor plus calcium channel blocker for the last 2 years. Imaging studies revealed narrowing of the right renal artery and external compression, caused by a giant hydatid cyst. The giant hydatid cyst was treated by cyst aspiration and alcohol injection. Despite cessation of angiotensin converting enzyme inhibitor plus calcium channel blocker, she remained normotensive on follow-up visits. New onset and resistant hypertension cases in elderly should be carefully evaluated for secondary causes of hypertension, including renovascular conditions. Mass effect of the hydatid cyst disease should be considered in the differential diagnosis.Öğe A comparative study of bimatoprost and travoprost: Effect on intraocular pressure and ocular circulation in newly diagnosed glaucoma patients(Karger, 2008) Alagöz, Gürsoy; Gürel, Kamil; Bayer, Atilla; Serin, Didem; Çelebi, Serdal; Kükner, ŞahapBackground: This study compares intraocular pressure (IOP)-lowering efficacy and retrobulbar hemodynamic effects of bimatoprost and travoprost in patients with newly diagnosed open-angle glaucoma. Methods: Patients were randomly assigned to one of two treatment groups ( bimatoprost group, n = 36; travoprost group, n = 46). IOP levels were measured with Goldmann applanation tonometer. Peak systolic velocity, end-diastolic velocity and resistivity index were obtained for each vessel by color Doppler imaging. Results: Both bimatoprost and travoprost significantly lowered IOP on days 30, 90 and 180 (p < 0.001). There was no significance between the 2 drugs on all follow-up visits. End-diastolic velocity of central retinal artery on day 180 was significantly higher than the value obtained at baseline in both groups. Conclusions: Patients were likely to achieve and maintain low target IOP with both drugs. Both drugs also resulted in improvement in the central retinal artery blood flow. Copyright (c) 2008 S. Karger AG, Basel.Öğe Comparison of the magnetic resonance imaging and knee arthroscopy findings of the menisci and ligamentous injuries(Duzce University Medical School, 2012) Özturan, Kutay Engin; Yücel, Istemi; Çakici, Hüsamettin; Güven, Melih; Gürel, KamilPurpose: Magnetic resonance imaging is frequently used in the diagnosis of the meniscal and ligamentous injuries. The aim of this study is to compare the results of the knee arthroscopy and magnetic resonance imaging findings retrospectively. Methods: Of the sixty-two patients 37 (59.7%) were male and 25 (40.3%) were female. The average age was 36.8 years (range 18-69 years). Patients who had been clinically diagnosed with meniscal and/or ligamentous injuries and subsequently undergone further magnetic resonance imaging examination were included to the study. The left knee was involved in 35 (56.5%) and the right knee in 27 (43.5%) cases. Patients had gone to knee arthroscopy according to clinical examination and magnetic resonance imaging findings, and the results were evaluated. Results: 35 (56.4%) of the 62 patients had isolated medial meniscal tear, 11 (17.7%) isolated lateral meniscal tear and 5 (8%) had isolated complete anterior cruciate ligament injury. 2 (3.2%) had medial meniscal tear and anterior cruciate ligament complete injury, 2 (3.2%) had lateral meniscal tear and complete anterior cruciate ligament injury and 5 (8%) had medial and lateral meniscal tear. 2 (3.2%) had medial meniscal tear, anterior and posterior cruciate ligament complete injuries. Magnetic resonance imaging evaluation of the medial meniscus, lateral meniscus, anterior cruciate ligament and posterior cruciate ligament revealed sensitivity 95.5%, 72.2%, 90.9%, 100%, specificity 76.4%, 93.1%, 88.2%, 98.3%, positive predictive value 91.4%, 81.2%, 62.5%, 66.6%, negative predictive value 86.6%, 89.1%, 97.8%, 100%, and accuracy 90.3%, 87.0%, 88.7%, 98.3% respectively. The purpose of this study was to determine the sensitivity of renal color Doppler sonography in differentiating obstructive and nonobstructive urinary calculi in patients with acute renal colic and to compare findings with nonenhanced helical computed tomography (CT). Methods. Sixty-five patients referred to the emergency department with acute renal colic underwent nonenhanced CT and renal resistive index (RI) measurement with color pulsed Doppler sonography within 8 to 10 hours of the onset of the symptoms. Computed tomographic evaluation was based on the detection of urolithiasis and classification according to location and the presence of obstruction. The mean RI of each kidney and the difference between the mean RI (Delta RI) of both kidneys were calculated and compared with CT findings. Results. A total of 164 stones were identified. Computed tomography revealed obstruction in 33 patients. Mean RI values for the obstructive and nonobstructive groups were 0.64 and 0.63, respectively. Mean Delta RI values were 0.01312 and 0.01000 in the obstructive and nonobstructive groups. The differences in the mean RI and Delta RI for the patients with and without obstruction were statistically insignificant (P =.73). No significant relationship was found between the RI values, calculus location, and degree of obstruction. Conclusions. Ultrasonography with high-frequency transducers is recommended as the first-line method for localization. We used automated injection of contrast agent with a power injector makings this application simpler and faster, achieved to show the iliac vein compression syndrome and post-thrombotic changes of the left iliac and femoral veins.Öğe Does an extra kidney-ureter-bladder radiograph taken in the upright position during routine intravenous urography provide diagnostic benefit?(Aves, 2008) Gürel, Kamil; Gürel, Safiye; Kalfaoğlu, Melike; Yılmaz, Özlem; Metin, AhmetPURPOSE The aim of this prospective study was to assess the diagnostic benefit of taking a kidney-ureter-bladder (KUB) radiograph in an upright position during routine intravenous urography (IVU). MATERIALS AND METHODS Between February 2005 and September 2007, 170 consecutive patients were included in the study. A basal IVU exam consisted of pre-contrast supine KUB, post-contrast supine KUB at the 7th and 15th minutes, and supine pelvic radiographs with full bladder and post-voiding. When needed, additional compression and/or oblique radiographs were taken. In this study, for all patients, a post-contrast 15th minute upright KUB radiograph was added to IVU. Two consecutive radiographs taken at the 15th minute postcontrast in supine and upright positions were evaluated by consensus of 2 radiologists. Primary benefits were improved filling and emptying of the collecting system, and secondary benefits were nephroptosis and ascertaining diagnosis of phlebolith. RESULTS Of 170 patients, 337 kidneys and collecting systems (n = 168 right; n = 169 left) were examined. Improved filling, emptying of the collecting system, nephroptosis, ascertaining diagnosis of phleboliths were detected with the rates of 12.5%, 44.2%, 8.3%, and 3.2%, respectively. Improved filling was significant in the presence of hydronephrosis (P < 0.05) and ureterolithiasis (P < 0.05) on both sides. There was a positive correlation between both improved filling and presence of hydronephrosis, and improved filling and presence of ureterolithiasis. Emptying was significant in collecting systems that had no visible pathology on IVU (P < 0.05) on either side. CONCLUSION Upright KUB radiographs provide supplementary data about urine flow in terms of improved filling and emptying of the collecting system.Öğe Does diagnostic sonography alter thyroid and parathyroid hormone levels?(John Wiley & Sons Inc., 2008) Serin, Erdinç; Gürel, Kamil; Kıyan, Aysu; Çağlayan, Murat; Karavaş, ErdalPurpose. To investigate possible alterations in the levels of thyroid and parathyroid hormones (PTHs) and thyroglobulin (TG) in healthy individuals following diagnostic sonographic examination of the thyroid gland. Methods. Thirty healthy women with no pathologic findings underwent sonographic examination, followed 6 weeks later by a second examination involving a probe-only application (PA) with the ultrasound scanner switched off. Duration times were identical for both examinations. Blood was drawn before and after the 2 applications. Thyroid hormone, PTH, and TG levels before and after the 2 applications were compared, and the difference between the variations for each parameter in the first and second applications was assessed. Blood samples were taken before and after the sonographic examination and the PA, and the serum concentrations of sensitive thyrotropin, total and free thyroxine, total and free tri-iodothyronine, TG, and PTH were measured. The pre- and post-examination levels of the hormones for the 2 applications were then compared and the difference between the variations for each parameter in the first and second application was then assessed. Results. The only significant variations observed were in the TG levels after PA and PTH levels after both sonographic examination and PA. The comparison between the 2 difference values revealed no significant difference except for PTH. Conclusion. This preliminary report on the possible influence of sonographic examination of the thyroid on the serum levels of thyroid and parathyroid hormones suggests that gland secretions such as PTH may be affected by external factors, including ultrasound. Clinicians should be aware of alterations in hormone levels by external factors. (c) 2007 Wiley Periodicals, Inc.Öğe Does prebiopsy, nonsterile ultrasonography gel affect biopsy-site asepsis?(Springer, 2008) Gürel, Kamil; Karabay, Oğuz; Gürel, Safiye; Hildebolt, CharlesPurpose The purpose of this study was to determine the extent to which the use of nonsterile gel, prior to antiseptic procedures in ultrasonography (US)-guided percutaneous biopsies, results in contamination of the biopsy site. Materials and Methods Patients referred for US-guided percutaneous biopsies were included in this study. Transmission material used for US evaluation before biopsy-site antiseptic procedures were performed was either nonsterile gel or sterile saline. Patients were randomly assigned to two groups: nonsterile gel (n = 30) and sterile saline (n = 30). Before the transmission material was used and after antiseptic procedures were performed, microbial swabs of a 10-cm(2)-diameter area were obtained at the biopsy site. Swabs were also obtained from the gel, saline, and povidine-iodine. Inoculated specimen plates were incubated at 37 degrees C under aerobic conditions, and the numbers of colony-forming units recorded. Nominal logistic regression analysis was used to calculate the odds of postantisepsis bacterial growth (after antiseptic procedures were performed) based on group, gender, coincidental disease (diabetes, chronic renal failure, and malignancy), biopsy-site location (head and neck or breast and abdomen), and local factors (skin fold, skin tag, and hair). Results The following odds ratios (adjusted for the other variables) and their 95% confidence intervals were calculated: (1) group (2.9 [0.8-11.1]; p = 0.10); (2) gender (1.2 [0.3-5.2]; p = 0.78); (3) coincidental disease (7.6 [0.9-166.7]; p = 0.09); (4) biopsy site location (6.2 [1.4-31.3]; p = 0.02); and (5) local factors (7.0 [1.6-36.0]; p = 0.01). No bacterial growth occurred with swabs obtained from gel, povidine-iodine, or saline. The use of CAs was reported in other clinical procedures such as odontology, neurological surgery, maxillary surgery, orthopaedics, plastic surgery, etc. In this experimental study the authors carried out posterolateral spinal fusion in a rat model to test the usefulness of ethyl-cyanoacrylate (ECA) in fixation of allograft/autograft fusions. Material and methods: 25 Sprague-Dawley rats were randomized into two study groups. Bilateral posterolateral lumbar intertransverse process spinal fusion was performed with both autograft and allograft in both groups. ECA gel was dropped in the fusion area in a rat model in group II. Results: The fusion rates as determined by manual palpation were 75% in controls and 46% in the ECA group (p = 0.322). According to radiographic score, the spinal segment was considered to be fused radiographically in 66% of controls and in 46% of animals in the ECA group (p = 0.43). The mean histological scores were 5.58 ± 0.14 and 4.53 ± 0.18 for the control and treatment group, respectively (p = 0.001). The mean bone density of the fusion masses was 101.3 ± 2.5 in the control group and 92.0 ± 3.3 in the ECA treatment group (p = 0.044). Conclusions: Ethyl-cyanoacrylate appeared to retard the osteogenic fusion but was well tolerated and did not induce necrosis, allergic reaction, infection, necrosis or neurological deficit in a rat model of posterolateral spinal fusion.Öğe The effect of risedronate on posterior lateral spinal fusion in a rat model(Korean Neurosurgical Society, 2009) Gezici, Ali Rıza; Ergün, Rüçhan; Gürel, Kamil; Yılmaz, Fahri; Okay, Önder; Bozdoğan, ÖmerObjective : To evaluate the potential effects of risedronate (RIS) which shows a higher anti-resorptive effect among bisphosphonates, after a posterolateral lumbar intertransverse process spinal fusion using both autograft and allograft in a rat model. Methods : A total of 28 Sprague-Dawley rats were randomized into 2 study groups. A posterolateral lumbar intertransverse process spinal fusion was performed using both autograft and allograft in a rat model. Group I (control) received 0.1 mL of steril saline (placebo) and Group II (treatment) received risedronate, equivalent to human dose (10 mu g/kg/week) for 10-weeks period. Results : The fusion rates as determined by manual palpation were 69% in the group I and 46% in the group II (p = 0.251). According to radiographic score, the spinal segment was considered to be fused radiographically in 7 (53%) of the 13 controls and 9 (69%) of the 13 rats treated with RIS (p = 0.851). The mean histological scores were 5.69 +/- 0.13 and 3.84 +/- 0.43 for the control and treatment groups, respectively. There was a significant difference between the both groups (p = 0.001). The mean bone density of the fusion masses was 86.9 +/- 2.34 in the control group and 106.0 +/- 3.54 in the RIS treatment group. There was a statistical difference in mean bone densities of the fusion masses comparing the two groups (p = 0.001). Conclusion : In this study, risedronate appears to delay bone fusion in a rat model. This occurs as a result of uncoupling the balanced osteoclastic and osteoblastic activity inherent to bone healing. These findings suggest that a discontinuation of risedronate postoperatively during acute fusion period may be warranted.Öğe The effects of chronic smoking on total cerebral blood volume measured by carotid and vertebral artery doppler ultrasonography(Wiley, 2017) Kalfaoğlu, Melike Elif; Hızal, Mustafa; Kıyan, Aysu; Gürel, KamilPurposeTo evaluate the effects of chronic cigarette smoking on total cerebral blood flow in healthy adults by Doppler ultrasonography (US). MethodsWe evaluated 50 smoker (median age 29) and 50 nonsmoker (median age 28) healthy, 20- to 40-year-old subjects without any cardiovascular and cerebrovascular disease. Peak systolic maximal blood flow velocity (PSV), end-diastolic maximal blood flow velocity (EDV), time-averaged mean blood flow velocity (TAMV), resistance index (RI), and pulsatility index (PI) were measured in the left and right carotid and vertebral arteries, and total cerebral blood flow volume was calculated. ResultsThere was no significant difference of smoking rate between genders. Blood pressure and PSV values were similar in both groups. EDV values of internal carotid artery (ICA) and vertebral artery (VA) were lower and RI and PI values were higher in smokers. TAMV, total ICA (-10.8%) and VA (-6%) flow volume, and tCBF (-9.2%) were lower in smokers. ConclusionsDoppler US is an effective tool to detect tCBF volume decrease in chronic cigarette smokers. Although minimal, this decrease, as demonstrated here in asymptomatic, healthy people, might be critical in patients with subclinical cerebral arterial insufficiency. (c) 2017 Wiley Periodicals, Inc. J Clin Ultrasound45:561-565, 2017Öğe Endovascular treatment of infrarenal abdominal aortic lesions with or without common iliac artery involvement(Springer, 2013) Önder, Hakan; Oğuzkurt, Levent; Gür, Serkan; Tekbaş, Güven; Gürel, KamilTo evaluate the results of stent placement for obstructive atherosclerotic aortic disease with or without involvement of the common iliac artery. Forty patients had self-expanding stents primarily or after balloon dilatation in the abdominal aorta between January 2005 and May 2011. All patients had trouble walking. Follow-up examinations were performed with clinical visits; these included color Doppler ultrasonography and computed tomographic angiography. Technical, clinical, and hemodynamic success was achieved in all patients. None of the patients underwent reintervention during the follow-up period, which ranged from 3 months to 6 years (median 24 months). Nine complications occurred in six patients. Of the nine complications, four were distal thromboembolisms, which were successfully treated with catheter-directed thrombolysis or anticoagulation therapy. Endovascular treatment of the obstructive aortic disease using self-expanding stents was safe and effective, with high technical success and long-term patency. Thromboembolic complications were high even though direct stenting was considered protective for thromboembolism formation. Particularly for infrarenal aortic stenosis, it can be recommended as the first-line treatment option for patients with obstructive atherosclerotic aortic disease.Öğe Endovascular treatment of surgically implanted arterial graft thrombosis by using manual aspiration thrombectomy(Turkish Soc Radiology, 2013) Önder, Hakan; Oğuzkurt, Levent; Özkan, Uğur; Gürel, Kamil; Gür, SerkanThe purpose of this study was to present our experience with guiding catheters in manual aspiration thrombectomy of occluded infra-aortic bypass grafts. This material was designed as a guiding catheter but was also used for thrombus aspiration. Six consecutive patients (all male; mean age, 61.0 +/- 5.7 years; range, 54-68 years) who underwent manual aspiration thrombectomy at the discretion of the operator for infra-aortic bypass graft thrombosis between 2002 and 2010 were retrospectively reviewed. The angiographic success described as either stenosis or residual thrombus less than 30% was 67%. Primary patency was 50%, and secondary patency was 66.7%. Additional stents were needed in four lesions of three patients. Manual aspiration thrombectomy is intended to remove both soft acute blood clots and hard organized embolic and thrombotic obstructions. Manual aspiration thrombectomy appears to be a safe and effective method for treating delayed graft thrombosis. This method provides an alternative to surgical thrombectomy, especially for patients who are not good candidates for the surgery.Öğe Geniculo-temporo-parotideal neurofibroma of the facial nerve: A case report(Centauro SRL, 2006) Yılmaz, Fahrettin; Gürel, Kamil; Gürel, Safiye; Sessiz N.; Boran, ÇetinPreoperative diagnosis of facial nerve neurofibroma is difficult when it presents as an asymptomatic parotideal mass. Facial nerve tumor suspicion arises during parotid surgery, histopathologic evaluation confirms diagnosis and postoperative imaging demonstrates a nerve tumor and its extension. We present the multimodality imaging findings of a 43-year-old man with a solitary neurofibroma involving the whole facial nerve continuously from geniculate ganglion to parotideal segment as the first case in the literature.
