Özet
Amaç: Hipokrom mikrositer eritrositleri olan çocuklarda HbA2 düzey tayiniyle b-talasemi taşıyıcı sıklığını araştırmak. Yöntem: Düzce merkez ve ilçelerinde okuyan 1,148 ilköğretim 8. sınıf öğrencisinden kan örnekleri alındı. Alınan örneklerin 122'sinde eritrosit ortalama volümü (EOV: MCV) < 79 fL veya eritrosit ortalama hemoglobin (EOH: MCH) < 27 pg saptandı ve b-talasemi taşıyıcılığı araştırıldı. Bu örneklerde high-performance liquid chromatography (HPLC) yöntemi ile hemoglobin A2 (HbA2) tayini yapıldı. Ayrıca, hipokromi ve/veya mikrositer örneklerden b-talasemi taşıyıcısı ve taşıyıcı olmayanlar kendi içinde iki gruba ayrıldı ve test sonuçları karşılaştırıldı. Bulgular: HbA2 düzeyi 7 (% 0.6) vakada % 3.5'e eşit veya yüksekti ve b-talasemi taşıyıcısı kabul edildi. b-talasemi taşıyıcı olan grup taşıyıcı olmayanlarla kıyaslandığında; taşıyıcı grubun ortalama MCV (p< 0.001), MCH (p< 0.001), Mentzer indeksinin (p< 0.001) daha düşük, eritrosit sayısı (p< 0.001) ve HbA2 (p< 0.001) değerlerinin ise daha yüksek olduğu saptandı. Sonuç: Batı-Karadeniz bölgesinde yer alan Düzce'de gerçekleştirdiğimiz bu ön çalışmayla, Düzce'de b-talasemi taşıyıcılığı sıklığının Türkiye geneline göre daha düşük olduğu sonucuna varılmıştır. Tarama programlarının etkinliğini arttırmak için, MCV ve MCH değerlerinin yanısıra RBC ve Mentzer indeksi kullanışlı parametreler olabilir.
Aim: To determine the prevalence of beta-thalassemia trait by measuring HbA2 in children with hypochromic microcytic erythrocyte in Duzce province and region. Methods: A total of 1,148 blood samples were obtained from eighth grade school children in central Duzce and surrounding seven towns. One hundred twenty-two out of 1,148 blood samples, which were investigated for beta-thalassaemia trait, showed either a mean corpuscular volume (MCV) < 79 fL or a mean corpuscular haemoglobin (MCH) < 27 pg. Hemoglobin A2 (HbA2) percentage was measured by high-performance liquid chromatography (HPLC) in these cases. Furthermore, individuals with microcytosis and/or hypochromia (n=122) were divided into two groups as beta-thalassemia carrier (n=7) and noncarrier (n=115), and test results were compared with each other. Results: In 7 subjects, the level of HbA2 was ?3.5 % and were considered beta-thalassemia trait. In the beta-thalassemia trait group, the mean MCV (p< 0.001), MCH (p< 0.001), and Mentzer index (p< 0.001) were lower, whereas the mean values of RBC (p< 0.001), and HbA2 percentage (p< 0.001) were greater compared to the noncarrier group. Conclusion: This study indicates that the prevalence of beta-thalassemia trait is lower in Duzce than that seen in Turkey as a whole, 0.6 % vs. 2.1 %, respectively. In addition MCV and MCH levels, RBC and Mentzer index can be used to improve the efficacy of the screening program.
