Efficacy of donepezil on cognitive functions in mild cognitive impairment

dc.description.abstractAmaç: Karmaşık mesleki ve sosyal görevleri olumsuz yönde etkileyecek, klinik olarak fark edilebilir düzeyde bilişsel bozulma ile giden durum, hafif kognitif bozukluk olarak bilinmektedir. Tedavide asetilkolinesteraz inhibitörlerinin kullanılması yakın zamana rastlamaktadır. Bu ilaçların kognitif fonksiyonlar üzerine etkinliği az sayıdaki çalışmayla ortaya konmuştur. Bu çalışmanın amacı, hafif kognitif bozukluğu olan hastalarda Donepezil’in kognitif fonksiyonlar üzerine etkisini araştırmaktı r. Yöntem: Bu çalışma 51 kişiden oluşmuştu. Randomize olarak seçilen 26 hasta çalışma grubunu, 25 hasta ise kontrol grubunu oluşturdu. Çalışma grubuna 24 hafta süreyle Donepezil HCl verildi. Kontrol grubuna ise herhangi bir psikotrop ilaç verilmedi. Bütün hastaların kognitif fonksiyonlar ilk çalışmaya alındığında, 4 hafta sonra ve 24 hafta sonra Wechsler Memory Scale-Revised’ın (WMS-R) yedi alt ölçeği ile değerlendirildi. Bulgular: Donepezil alan grup ile kontrol grubu arasında şekil belleği alt ölçeği, sözel çağrışım çiftleri alt ölçeği ve mantıksal bellek alt ölçe- ği açısında fark istatistiksel olarak anlamlıydı (p<0,001). Çalışma grubunda sözel çağrışım çiftleri ve mantıksal bellek skorları iyileşmiş bulundu ancak şekil belleğinde azalma tespit edildi. Sonuçlar: Donepezil hafif kognitif bozukluğu olan hastalarda özellikle hafıza ve dikkat üzerinde düzelmeye sebep olmuştur. fiekil belleği skorundaki düşüş Donezepilin beynin her bölgesinde aynı etkiyi göstermemesiyle açıklanabilir.en_US
dc.description.abstractObjective: The condition characterized by clinically distinctive cognitive impairment and likely to affect occupational and social functioning is known as Mild Cognitive Impairment (MCI). Acetylcholinesterase inhibitors have been used for the treatment of MCI up to now. However, there have been few studies to show the efficacy of these drugs on cognitive functions. The aim of this study was to investigate the effects of donepezil on cognitive functions in patients with MCI. Method: This study included 51 patients. 26 patients were randomized to the study group and 25 to the control group. The study group received donepezil HCl for 24 weeks. The control group did not receive any psychotropic drugs. Cognitive functions of all patients were evaluated by seven subscales of the Wechsler Memory Scale- Revised (WMS-R) at baseline, 4th week and 24th week of the study. Results: There was a significant difference in figural memory, verbal paired associates and logical memory subscale scores between the groups (p&lt;0,001). Although there was an improvement in verbal paired associates and logical memory subscale scores in the study group, there was a decrease on the figural memory test scores. Conclusion: It was found that donepezil led to an improvement especially in memory and attention in MCI. The decrease in figural memory scores can be explained by the fact that donepezil does not exert the same effect on all areas of the brain.en_US
