HBV DNA düzeyleri ile HBV serolojik göstergeleri arasındaki ilişkinin araştırılması

dc.description.abstractAmaç: Bu çalışmada akut veya kronik hepatit ön tanılı hastalarda HBV DNA pozitifliği ile HBsAg, HBeAg ve Anti-HBe arasındaki ilişkinin araştırılması amaçlanmıştır. Gereç ve Yöntemler: Mart 2010 ile Temmuz 2012 arasında mikrobiyoloji laboratuvarına serum örnekleri gönderilen 574 hastaya ait veriler geriye dönük olarak incelenmiştir. Bulgular: Çalışmamızda HBsAg pozitif hastaların %44,8’inde, HBeAg pozitif hastaların %67,1’inde, anti-HBe pozitif hastaların %35,9’unda HBV DNA saptanmıştır. HBsAg ve HBeAg birlikte pozitif olan olguların %61,2’sinde HBV DNA saptanırken; her iki belirtecin negatif sonuçlandığı 29 hastanın birinde (%3,4) HBV DNA tespit edilmiştir. HBV DNA pozitifliği, HBsAg negatif hastalarda %6,3; HBeAg negatif hastalarda ise %37,7 olmuştur. Çalışmamızda HBV DNA saptanan hastaların %79,4’ünde HBeAg negatif olarak belirlenmiştir; HBV DNA pozitif hastaların %17,7’sinde Anti-HBe saptanmamıştır. HBeAg pozitifliğinde HBV DNA miktarı anlamlı olarak yüksek bulunmuştur (p<0,001). Anti-HBe negatif bulunan hastalardaki HBV DNA miktarı anlamlı oranda yüksek saptanmıştır (p<0,001). Sonuç: Sonuç olarak, serolojik göstergeler tek başına hepatit B enfeksiyonu tanısında ve HBV replikasyonunu belirlemede yetersiz kalabilmektedir. Bu çalışmada hepatit B enfeksiyonunun laboratuvar tanısında, HBV DNA’nın kantitatif saptanmasının faydalı olduğu görülmüştür.en_US
dc.description.abstractObjective: In this study, it is aimed to investigate the relationship between the HBV DNA positivity and serological markers such as HBsAg, HBeAg and anti-HBe in patients with acute or chronic hepatitis. Materials and Methods: Data of 574 patients whose serum specimens were processed in microbiology laboratory between march 2010 and july 2012 were retrospectively analysed. Results: In the study, HBV DNA was detected in 44.8% of HBsAg-positive, in 67.1% of HBeAg-positive and in 35.9% of anti-HBE-positive patients. HBV DNA was detected in 61.2% of the patients both of whose HBsAg and HBeAg were positive. However, HBV DNA was positive in one of 29 patients (3.4%) who had negativity of both HBsAg and HBeAg. DNA positivity was 6.3% in HBsAg-negative patients and 37.7% in HBeAg-negative ones. In the study, HBeAg was negative in 79.4% of HBV DNA-positive patients; anti-HBeAg was negative in 17.7% of DNA-positive patients. DNA levels were found significantly high in HBeAg-positive patients (p<0.001) as well as DNA levels were significantly high in anti-HBe- negative ones (p<0.001). Conclusion: In conclusion, serological markers may be insufficient either in diagnosis of HBV infection or in determining the viral replication. In this study, it is observed that evaluating those tests together is useful in laboratory diagnosis.en_US
