Kompanse kronik böbrek hastalığı olgularında nt-pro bnp ile atrial ileti süreleri arasındaki ilişkinin değerlendirilmesi

Özet
Amaç: Bu çalışmanın amacı kompanse dönemdeki kronik böbrek hastalığı olgularında NT-pro BNP ile atrial ileti süreleri arasındaki ilişkinin değerlendirilmesidir. Gereç ve yöntem: Çalışmamızda belirli dışlama kriterleri göz önünde bulundurularak altmış kompanse kronik böbrek hastalığı (KBH) olgusu alındı. Olguların NT-pro BNP değerleri ve rutin biyokimyasal laboratuvar testleri çalışıldı. Konvansiyonel ve doku doppler ekokardiyografi yöntemi ile rutin ekokardiyografik parametreleri ve atrial ileti süreleri hesaplandı. İstatistiksel olarak aralarındaki ilişki değerlendirildi. Bulgular: NT-pro BNP ile sol intraatrial ve interatrial ileti süreleri arasında pozitif anlamlı bir ilişki saptandı. NT-pro BNP ile sol ve sağ atriyum çapları, interventriküler septum kalınlığı, arka duvar kalınlığı, tekrar ve vena kava inferior inspiratuar ve ekspiratuar çapları arasında anlamlı düzeyde bir pozitif korelasyon saptandı. Sonuç: Kompanse KBH hastalarında NT-pro BNP ile atrial ileti süreleri arasındaki ilişkiyi değerlendirdiğimiz bu çalışmamızda hipervolemiye bağlı olarak hem NT-pro BNP'de artış hem de atrial ileti sürelerinde uzama olduğunu ve bunların aralarında anlamlı bir ilişki olduğunu saptadık. KBH ve KVH'lar birbirinden ayrılamayan birbirini tetikleyen iki önemli hastalık grubu olup böyle olguların erken tespiti ve etkin tedavisi önemlidir. Aim: The aim of this study is to identify the association between N terminal pro brain natriuretic peptid (NT-pro BNP) and atrial conduction times and mechanical functions in predialysis chronic kidney disease (CKD) patients. Materials and methods: Sixty compensated chronic kidney disease patients enrolled in this study. NT-pro BNP levels and other routine biochemical laboratory parameters are studied. Conventional echocardiography performed to obtain standard echocardiographic parameters. Atrial conduction times are measured by tissue doppler echocardiography. The associations between results were analysed. Results: NT-pro BNP levels were correlated with left intraatrial and interatrial conduction times significantly. Similarly, the same correlation was detected between NT-pro BNP levels and standard echocardiographic parameters. Another important result of this study was evidence of a significant relationship demonstrated between left intraatrial conducting time and standard echocardiographic parameters which predicts hypervolemia. Discussion: To our knowledge this is the first study in literature which identified the relationship between NT-pro BNP and atrial conduction times and mechanical functions in the predialysis CKD patients. According to present results, we think that the increase in NT-pro BNP, which is considered as marker of hypervolemia, may be a consequence of deterioration in cardiac electrophysiologic parameters in CKD patients. We also claim that, hypervolemia, which worsen cardiovascular prognosis, may be a common physiopathological consequence of atrial conduction abnormalities and deterioration in echocardiographic parameters.
