Özet
Amaç: Fakoemülsifikasyon ile saydam lens ekstraksiyonu (SLE) uygulanan yüksek miyop olguların sonuçlarının değerlen- dirilmesi. Gereç ve Yöntem: Kliniğimizde 2005-2011 yılları arasında refraktif amaçlı SLE uygulanan 15 yüksek miyop olgunun 25 gözü retrospektif olarak incelendi. Olgular görme keskinliği, refraksiyon, intraoperatif ve postoperatif komplikasyonlar ve arka kapsül opaklaşması açısından değerlendirildi. Bulgular: SLE uygulanan 15 miyop hastanın (6 erkek, 9 kadın) yaş ortalaması 53.08±12.42 (36-80) yıldı. Preoperatif ve posto- peratif ortalama sferik eşdeğer refraksiyon -14.90±5.15 diyoptri (D) ve -0.67±0.94 D idi (p<0.01). Preoperatif ve postoperatif en iyi düzeltilmiş görme keskinliği ortalamaları sırasıyla 0.55±0.48 ve 0.24±0.23 (LogMAR) idi (p<0.01). Hiçbir olguda intra- operatif komplikasyon izlenmedi. Bir gözde postoperatif 13. ayda retina dekolmanı, 2 gözde arka vitre dekolmanı ve 1 gözde postoperatif göz içi basıncı artışı gelişti. Bir gözde biyometri sapmasına bağlı -1.50 D’lik hata gözlendi. Onbeş gözde (%39.47) değişik derecelerde arka kapsül kesafeti saptandı. Bu gözlerden 11’ine (%73.3) belirgin görme düşüklüğü nedeniyle Nd:YAG lazer arka kapsülotomi yapıldı. Lazer kapsülotomi yapılan olgulardan birinde retina dekolmanı gelişti. Tartışma: Saydam lens cerrahisinin görsel ve refraktif sonuçları yüz güldürücü olmakla birlikte, ameliyat sonrası retina de- kolmanı yüksek oranda görülebilmektedir. Bu nedenle her hastanın olası komplikasyonlar açısından bilgilendirilmesi, cerrahi öncesi oldukça ayrıntılı değerlendirilmesi ve postoperatif dönemde yakından takip edilmesi gerekmektedir.
Purpose: To evaluate the surgical outcomes of clear lens extraction (CLE) using phacoemulsification in high myopic eyes. Material and Methods: In this retrospective study, we evaluated the records of 15 patients with high myopia who under- went CLE between 2005 and 2011 in our clinic. Main outcome measures were visual acuity, refraction, intraoperative and postoperative complications, and posterior capsular opacification (PCO). Results: 25 eyes of 15 patients (6 men, 9 women) with a mean age of 53.08±12.42 (36-80) years who undervent CLE were evaluated. Mean preoperative and postoperative spherical equivalent refraction were 14.90±5.15 Diopters (D) and -0.67±0.94 D (p<0.01), respectively. Preoperative and postoperative best corrected visual acuities were 0.55±0.48 and 0.24±0.23 (Log- MAR), respectively. No patients had intraoperative complications. During follow-up one eye had retinal detachment at postoperative 13th months, two eyes had posterior vitreous detachment, and one eye had increased intraocular pressure postoperatively. Due to biometry error, one eye showed -1.50 D refractive shift. Various degrees of PCO were observed in 15 eyes (39.47%) during the follow-up period. Nd:YAG laser capsulotomy was performed 11 of those eyes (73.3%). One patient had retinal detachment after laser capsulotomy. Conclusion: Although the visual and refractive results of clear lens extraction are satisfactory, high incidence of postop- erative retinal detachment can be observed. Therefore, all patients should be informed because of possible postoperative complications, evaluated in detail preoperatively and closely monitored postoperatively.
