ÖZET DİZ OSTEOARTRİTLİ HASTALARIN GÜNLÜK YAŞAMLARINA İLİŞKİN YARDIM GEREKSİNİMLERİNİN BELİRLENMESİ VE DEĞERLENDİRİLMESİ Cerit (Tepe) Birgül Yüksek Lisans, Hemşirelik Bölümü Tez Danışmam: Doç. Dr. Feray Gökdoğan OCAK 2003, 112 sayfa Bu araştırma, diz osteoartritli hastaların günlük yaşamlarına ilişkin yardım gereksinimlerinin belirlenmesi ve değerlendirilmesi amacıyla yapılmıştır. Araştırmanın örneklemini Bolu İli Fizik Tedavi ve Rehabilitasyon Hastanesi polikliniğine 4 Temmuz- 4 Ağustos 2002 tarihleri arasında başvuran 103 hasta oluşturmuştur. Veriler, araştırmacı tarafından konu ile ilgili literatür incelenerek ve uzman görüşü alınarak hazırlanan soru formu aracılığıyla elde edilmiştir. Veriler yüzdelik, ki-kare, t testi, One Way Anova yöntemleri kullanılarak analiz edilmiştir. Bolu İli Fizik Tedavi ve Rehabilitasyon Hastanesi'ne başvuran diz osteoartritli hastaların çoğunluğunun 50-59 yaş (% 33.0) ve 60-69 yaş grubunda (% 34.0) yer aldığı, % 79.6'sı kadın olup, % 51.4'ünün okur-yazar ve ilköğrenim düzeyinde eğitim aldığı, % 95.1 'inin çalışmadığı ve tamamına öt»»* «.SESİ* ¦^sss»***yakınının (% 99.0) sosyal güvencesi olduğu, alt gelir düzeyinde (% 65.1) olup, eşiyle (% 39.8) ile birlikte yaşadığı belirlenmiştir. Kadın hastaların dizlerine ilişkin "şiddetli" ve erkeklerin ise "orta" düzeyde sıkıntı yaşadıkları saptanmış ve bu fark anlamlı bulunmuştur (X2: 26.061; PO.05). Diz osteoartritli erkek hastaların günlük yaşam aktivitelerindeki yeterlilik düzeyleri kadınlara göre daha yüksek bulunmuştur. Çorap giyme/çıkarma aktivitelerini kadın (4.82±2.79) ve erkek hastalar (5.9ܱ2.83) en üst yeterlilik düzeyinde tanımlamışlardır. Hastaların büyük çoğunluğunun (% 63.1) ayakta yapılan işlerde zorluk çektikleri ve yaklaşık yansının (% 51.5) yardıma gereksinim duyduğu belirlenmiştir. Diz osteoartritli hastaların tamamına yalanının (%94.2) yorgunluk hissettikleri, bu oranın kadınlarda (% 97.6), erkeklere göre (% 81.0) daha fazla olduğu saptanmıştır (X2: 8.406, P< 0.05). Kadın ve erkeklerin çoğunluğunun "her zaman" ve "çoğu zaman" yorgunluk hissettikleri; kadın hastaların iş yaparken hissettikleri yorgunluk düzeyinin (10 birimlik cetvel üzerinden) (8.31Ü.80) erkeklere göre (6.29±1) daha yüksek olduğu ve bu farkın anlamlı olduğu saptanmıştır (t: 4.291; P<0.05). Diz osteoartritli hastaların yaklaşık tamamının ağrı yaşadığı (% 98. 1); ağrı yaşayan erkeklerin yaş ortalamalarının (64.35±7.95) kadınlardan daha yüksek (60.05±10.09) olduğu; kadın hastaların "sürekli" ağrı hissettikleri (% 46.9) erkek hastaların ise ağrılarının "bir saat kadar" (% 60.0) sürdüğü, hastaların yandan fazlasının (% 52.5) en fazla ağrıyı ayakta yaptıkları işlerde hissettikleri; bu ağrıla rı azaltmak için çoğunlukla ilaç aldıkları (% 68.3) belirlenmiştir. vııKadınların (% 63.4) erkeklerden (% 33.3) daha fazla oranda uyku sorunu yaşadıkları (X2: 6.182, P<0.05); kadınların yaş ilerledikçe (60.6718.04) uyku sorunu yaşama durumlarında artış olduğu belirlenmiştir. Sabah katılığı yaşayan diz osteoartritli hastaların çoğunluğu (% 72.8) oluşturduğu; kadınların (% 46.9) ve erkeklerin (% 54.5) çoğunlukla "şiddetli" düzeyde sabah katılığı yaşadıkları; ancak "şiddetli" düzeyde sabah katılığı yaşayan erkeklerin kadınlara göre daha fazla olduğu saptanmıştır (X2:12.221, P<0.05). Diz osteoartritli hastaların yaklaşık yansının hastalıkları nedeniyle iletişimlerinin etkilendiği (% 44.7) ve etkilenmediği (% 55.3) öğrenilmiştir. Diz osteoartritli hastaların hastalıkları nedeniyle gerçekleştiremedikleri, ancak en çok yapmak istedikleri üç şey olarak; rahat/mutlu olmanın (% 38.5) ilk sırada geldiği, daha sonra sırasıyla bağımsız olmayı (% 30.1) ve hızlı iş/hareket etmeyi (%18.8) istedikleri öğrenilmiştir. ABSTRACT SPECIFICATION AND EVALUATION OF THE NEEDS OF THE PATIENTS WITH KNEE OSTEOARTHRITIS IN THEIR DAILY LIVING ACTIVITIES Cerit (Tepe), Birgül M, Sc, Department of Nursing Supervision Assoc. The sampling as selected from the Hospital of Physiotherapy and Rehabilitation in Bolu, it includes 103 patients applying to the hospital between 4th July and 4th August 2002, as the subjects to the study. The data has been gathered by applying to the expert views and reviewing the relevant literature. The data is analyzed through proportional, chi suquare, t- test and one-way Anova. The subjects, the patients with knee osteoarthritis applying to the hospital stated, are majorly between the ages 50-59 (% 33.0) and 60-69 (% 34.0), % 79.6 of all female, and % 51.4 are literate and elementary school qraduates, %95.1 are housewives and almost all have social assurance (%99.0). They are almost all, from low-income social class (% 65. 1) and live with their husbands (% 39.8). inThe problem of the female patients with their knees has been found to be "severe" and of the male patients to be "moderate" and this difference has been found to be significant (X2:26, 061; p<0.05). The sufficiency level in daily activities of the male patients with knee osteoarthritis shows higher than that of female; their activities of taking on/off their socks are found to be (4.82± 2.79) and the activities of the wales, (5.90 ± 2.83) at the highest sufficiency level. A large percent of the patients are observed that they have trouble with the activities as they are up (% 63.1) approximately half of them need assistance (% 51.5). The patients with knee osteoarthritis, almost all, feel tired (%94.2). This rate is observed to be higher (% 97.6) in females than males (% 81.0), (X2:8.406; p<0.05). Most of the both male and female patients are attributed the feeling of tired with the scales "always" and "often". This feeling, on a 10 unit measurement scale, has been observed to be higher in females (8.31 ± 1.80) as compared with that of males (6.29 ± 1), (t:4.291, p<0.05). Also, it is found, that almost all of the patients feel pain (% 98.1), male patients with pain show the average age (64.35 ± 7.95), which is higher than the average age of females (60.05 ± 10.09). The female patients (% 46.9) "always" feel the pain; on the other hand male patients feel the pain for "an hour" (% 60.0), more than the half are observed to feel the pain as they are in up position (% 52.5). It is reported that they take medicine to relieve the pain (% 68.3). Another outcome öf the study is the insomnia experiewed by the female (% 63.4) than the male (% 33.3), (X2: 6.182; p<0.05). As the female patients get older (60.67 ± 8.04) the insomniac rate in this group grows. IVThe rate of the patients with knee osteoarthritis suffering "morning stiffness" (% 72.8) is found to be the major and of the female (% 46.9) and male (% 54.5) majorly suffer morning stiffness" "severe" however, the male patients suffering morning stiffness, are found to be much more than that of females, (X2: 12.22; p<0.05). It has been concluded that the patients with knee osteoarhritis almost half of them, are affected (44.7) in their communication due to the illness and not affected (% 55.3). It has also been learned that the patients with knee osteoarhritis, owing to their illness, would prefer doing three activities: experiending a comfortable and happy life (% 38.5), then being "independent (% 30.1) and having a fast movabilitiy (% 18.8).
