    Massive gastrointestinal tubercolsis in a young patient without immunosupression

    Although the lung is the major site for Mycobacterium tuberculosis infection, gastrointestinal involvement can be present as part of multiorgan disease process or, less commenly, can be seen as primary gastrointestinal tuberculosis. In the cases where the culture is negative, it can be difficult to differantiate tuberculosis from Crohn's disease based on both the clinical and histological features. When side effects of classic antimycobacteria are encountered, we can initially add ciprofloxacin to the treatment of tuberculosis. We reported a case of 19-yr-old patient, who was treated as Crohn's disease and worsen. We began to tuberculosis treatment, and the patient improved clinically and histologically. The main point in this case is that widespread involvement of gastrointestinal tract can be brought about by non resistant strains of Mycobacterium tuberculosis even in immunocompetent patients.

    Molecular analysis of isoniazid, rifampin and streptomycin resistance in Mycobacterium tuberculosis isolates from patients with tuberculosis in Duzce, Turkey

    The aim of this study was to use DNA sequencing analysis to analyze the mutations in the most commonly targeted genes (katG, inhA, rpoB, rpsL) in isoniazid (INH)-, rifampin (RIF)- and streptomycin (SM)-resistant Mycobacterium tuberculosis strains obtained from subjects in Duzce, Turkey. Four isolates were found to be INH-resistant, 3 were RIF-resistant and 5 were SM-resistant, out of a total of 52 M. tuberculosis strains. In 3 of the 4 INH-resistant strains, a mutation in the katG gene in codon 315 appeared as AGC -> ACC (Ser -> Thr), and the other INH-resistant strain showed a mutation in the katG gene in codon 314 as ACC -> CCC (Thr -> Pro). There were no mutations in the inhA gene in INH-resistant isolates. Two of the 3 RIF-resistant strains were found to have mutations in the rpoB gene in codon 516 appearing as GAC -> GTC (Asp -> Val), and the other RIF-resistant strain has a mutation in the rpoB gene in codon 531 as TCG -> TTG (Ser -> Leu). These 3 RIF-resistant strains are also INH-resistant. All 5 SM-resistant strains have mutations in the rpsL gene in codon 43 appearing as AAG -> AGG (Lys -> Arg). Thus, we found common gene mutations that bring about the resistance of M. tuberculosis to antituberculosis drugs in all of our isolates from Duzce. To the best of our knowledge, the ACC -> CCC (Thr -> Pro) mutation in the katG gene in codon 314 has not been previously defined.

    P wave duration changes and dispersion A risk factor or autonomic dysfunction in stroke?

    Objectives: To investigate the P wave duration and P wave dispersion in stroke patients, and to compare those with healthy subjects. Methods: We measured maximum and minimum P wave durations, and dispersion on the 12-lead surface ECG in 67 consecutive patients with first ever-acute ischemic stroke and 58 controls at the neurology wards of the Medical School, Duzce, Turkey, between May 2005 and October 2006. The subjects were not included if there were a history of atrial fibrillation, cardiac problem, and using drugs related to cardiovascular diseases or psychiatric problems. Results: P wave durations and dispersion were similar in stroke patients and controls. The correlation analysis revealed a positive relation between age and Pmin duration (p=0.03). The mean Pmin values were 63.85 +/- 22.55 for male and 76.43 +/- 26.84 for female patients, and this difference was statistically significant (p=0.04). The correlations between P wave durations, and the presence of risk factors, the stroke outcome scales, and death of patient within 6 months were not statistically significant. Conclusions: Although there were some previous reports on ECG changes including P waves in acute stroke, we found that P wave durations and dispersion were similar in acute stroke patients and controls. This may be related to the patient selection criteria of this study, as we did not include patients with any previous cardiac abnormality We concluded that the autonomic nervous system dysfunctions causing cardiac abnormalities in stroke need more investigation.

    Myopia in preterm children at 12 to 24 months of age

    Purpose: To examine biometric factors that may influence the high incidence of myopia in preterm children at 12 to 24 months of age. Patients and Methods: Fifty eyes of 25 patients with myopia and 70 eyes of 35 patients with emmetropia at 12 to 24 months of age were selected from a group of preterm children who had been screened for retinopathy of prematurity and returned for follow-up examinations. Forty eyes of 20 full-term children of the same age were also included in the study as a control group. A-scan ultrasonography was performed to measure axial length, lens thickness, and anterior chamber depth in the three groups. Results: The myopic group had a significantly greater axial length than did the other groups. However, the three groups did not differ in lens thickness or anterior chamber depth. The degree of myopia increased with the increase in axial length and the decrease in anterior chamber depth. Conclusions: The development of myopia in pre-term children 12 to 24 months of age appears to be influenced mainly by axial length, whereas the degree of myopia seems to be related to axial length and anterior chamber depth. Among refractive components, axial length plays a major role in myopia occurring at this age in preterm patients.

    Efficacy of throat gargling for detection of group a beta-hemolytic streptococcus

    The purpose of our study was to investigate the suitability of throat gargling with sterile saline as an alternative method to throat swabs for detection of group A beta-hemolytic streptococcus (GAS). Throat specimens were obtained from 601 cases belonging to different age groups. Sterile Dacron swabs and gargle residue were first streaked on the side of a 5% sheep blood agar plate to which a 0.04 U bacitracin disk bad been applied, and then 1.25 mg trimethoprim and 23.75 mg sulphamethoxazole were added to the plate. After incubation, beta-hemolytic colonies were classified serologically by latex agglutination. GAS was detected in both throat swabs and throat gargle specimens in 49 cases, but GAS was also detected in 12 throat swabs from patients with culture-negative gargles and in 8 gargle specimens from subjects in whom throat swabs were culture negative. The strength of agreement was evaluated by calculating the kappa coefficient (K = 0.82, P = 0.000). The sensitivity, specificity, positive predictive value, and negative predictive value of throat gargle specimens were 80.3, 98.5, 85.9, and 97.8%, respectively. Although the conventional throat swab culture remains the gold standard, the throat gargle method is a quick, safe, and easy method for detection of GAS that serves as an effective alternative to throat swab culture.

    Effects of depression and anxiety on quality of life of patients with rheumatoid arthritis, knee osteoarthritis and fibromyalgia syndrome

    Objective: To measure the effects of depression and anxiety on quality of life (QoL) in patients with rheumatoid arthritis (RA), knee osteoarthritis (OA) andfibromyalgia syndrome (FMS). Methods: One hundred and fifty-four patients with RA, knee OA, and FMS who presented to the physical medicine and rehabilitation department were studied For evaluation of the patients, Beck depression scale, Beck anxiety scale, and Short Form-36 were used. Results: Twenty-two per cent ofpatients (n = 34) were diagnosed with of RA, 52.6% (n = 81) knee OA and 25.3% (n = 39) FMS. Except for the subscales, of physical and emotional role, there were statistically significant differences among diagnostic groups in the rest of the SF-36 subscales. In the physical functioning subscale, the highest score was obtained in the fibromyalgia group and the lowest in the RA group (p < 0.001). However in the bodily pain subscale, the lowest score was recorded in the fibromyalgia group (p = 0.019). In all diagnostic groups, the scores of SF-36 subscales were significantly low in patients who scored above the threshold value of Beck depression scale (p < 0.001). A strong negative correlation was detected between scores of Beck anxiety scale and the scores of all SF-36 subscales in patients with RA and knee OA. On the other hand, in patients with FMS, anxiety scores correlated negatively with only physical and somatic function scores of SF-36. Conclusion: Quality of life is significantly low in patients with RA, knee OA and FMS, whose depression and/or anxiety scores are high. Therefore, these patients should be managed using a multidisciplinary approach including psychiatric support.

    Relation of neutrophil/lymphocyte ratio with the presence and extent of coronary artery ectasia

    Objectives: It has been shown that the neutrophil to lymphocyte ratio (N/L ratio) is associated with cardiovascular events and mortality. In this study, we investigated the N/L ratio in patients with coronary artery ectasia (CAE). Study design: Fifty patients (29 men, 21 women; mean age, 51.1+/-7.1 years) diagnosed as CAE using coronary angiography were included in the study. The control group consisted of 28 patients (16 men, 12 women; mean age, 49.5+/-9.4 years) who had normal coronary arteries as determined by coronary angiography. Baseline characteristics were recorded. The number of ectatic segments was noted. Hematologic parameters were measured and the N/L ratio was calculated. Results: The N/L ratio was significantly higher in the CAE group compared with the control group (median [25-75% percentile] 2.2 [1.6-3.0] vs. 1.8 [1.4-2.0], p=0.014, respectively).     Emotion Regulation, Theory of Mind, and Attachment to Parents and Peers Among Turkish Adolescent Offenders and Victims: A Single-Center, Cross-Sectional, Case-Control Study

    Background: There may be many risk factors for both youth offending and victimization. In our study, we aimed to compare youth offenders and victims in terms of attachment characteristics, emotion regulation, and mind-reading skills.Methods: This study employed a single-center, cross-sectional, case-control design. Kiddie and Young Adult Schedule for Affective Disorders and Schizophrenia Present and Lifetime Version along with diagnostic criteria of Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, were administered by clinicians. Mind-reading skills were assessed with Reading the Mind in the Eyes task (RMET) and Faces test. Difficulties in Emotion Regulation Scale (DERS) and Inventory of Parent and Peer Attachment-Short Form (IPPA-SF) were used for assessing attachment and emotion regulation properties.Results: When we compared the two groups in terms of IPPA-SF, DERS, RMET and Faces test, we found that victimized and offending youth did not differ significantly in terms of attachment to peers and communication/trust domains of parental attachment. Youth offenders and victims differed significantly in terms of DERS-Non-acceptance and Goals both (P = .031 and .045; respectively). Offending youth scored significantly higher in Non-acceptance, while victimized youth scored significantly higher in Goals.Conclusion: Offending youth were more alienated from their parents, were experiencing problems with emotional acceptance, and had lower theory of mind and emotion recognition skills compared to victimized youth. Therefore, protective interventions supporting parent-adolescent commu nicat ion/ attach ment, as well as emotion recognition/regulation and theory of mind skills of youth, may protect children from both victimization and delinquency.

    The comparison of laparoscopy, shock wave lithotripsy and retrograde intrarenal surgery for large proximal ureteral stones

    Introduction: In this study we compare the success rates and complication rates of shock wave lithotripsy (SWL), laparoscopic, and ureteroscopic approaches for large (between 1 and 2 cm) proximal ureteral stones. Methods: In total, 151 patients with ureteral stones between 1 and 2 cm in diameter were randomized into 3 groups (52 SWL, 51 laparoscopy and 48 retrograde intrarenal surgery [RIRS]). The groups were compared for stone size, success rates, and complication rates using the modified Clavien grading system. Results: Stone burden of the groups were similar (p = 0.36). The success rates were 96%, 81% and 79%, respectively in the laparoscopy, SWL, and ureteroscopy groups. The success rate in laparoscopy group was significantly higher (p < 0.05). When these groups were compared for complication rates, RIRS seemed to be the group with the lowest complication rates (4.11%) (p < 0.05). SWL and laparoscopy seem to have similar rates of complication (7.06% and 7.86%, respectively, p = 0.12). Interpretation: To our knowledge, this is the first study to compare the results of laparoscopy, SWL and RIRS in ureteral stones. Our results showed that in management of patients with upper ureteral stones between 1 and 2 cm, laparoscopy is the most successful method based on its stone-free rates and acceptable complication rates. However, the limitations of our study are lack of hospital stay and cost-effectiveness data. Also, studies conducted on larger populations should support our findings. When a less invasive method is the only choice, SWL and flexible ureterorenoscopy methods have similar success rates. RIRS, however, has a lower complication rate than the other approaches.

    THYROID-RELATED FACTORS THAT INFLUENCE PREOPERATIVE LOCALIZATION OF PARATHYROID ADENOMAS

    Objective: To evaluate the effect that thyroid-related factors have on the preoperative localization of parathyroid adenomas. Methods: This retrospective study included adult patients who were referred for further evaluation of primary hyperparathyroidism between December 2005 and October 2009 at a teaching and research hospital in Turkey. High-frequency ultrasonography and sestamibi scintigraphy (MIBI) were performed in all patients. Surgical procedure involved focal or bilateral exploration on the basis of concordant or discordant imaging studies. Selection of patients for minimally invasive parathyroidectomy was made based on the presence or absence of a single parathyroid adenoma detected by both ultrasonography and MIBI scan. Patients with negative or discordant imaging studies and a concomitant thyroid nodule underwent bilateral neck exploration. Results: Two hundred and forty-eight patients with primary hyperparathyroidism who underwent parathyroidectomy were included in the study. Parathyroid gland abnormalities were successfully detected preoperatively by ultrasonography in 231 patients and by MIBI scan in 152 patients. When used together, ultrasonography and MIBI scan were unsuccessful in detecting an abnormality in 11 cases. MIBI scan visualized a lesion in 6 cases that remained undiagnosed by ultrasonography. Fifty-six of 85 patients with lesions detected by ultrasonography, but not by MIBI scan, had thyroid nodules. The frequency of thyroid nodules was higher in the 96 patients in whom a MIBI scan could visualize a parathyroid lesion than in the 152 patients in whom MIBI scan was successful (P = .004). No difference was observed regarding ipsilateral thyroid lobe involvement or nodule volume. Parathyroid adenomas were significantly smaller in patients with negative MIBI scans (P<.001). Conclusion: Our results suggest that ultrasonography is more sensitive than MIBI scan in the detection of parathyroid adenomas, particularly in the presence of small parathyroid adenomas or other thyroid related-factors.     Entoptic Phenomenon Following Pars Plana Vitrectomy Caused by Retained Preretinal Gel

    A patient who underwent pars plana vitrectomy for vitreous floaters is described. At the time of surgery, the posterior hyalold was avulsed and stripped from the posterior retinal surface. Postoperatively, the patient described filamentous and sea-fan entoptic phenomena scattered throughout the periaxial vision that were in focus, attached, and waving with movement that was counter to the direction of gaze pursuit and demonstrated after movement. The authors believe these structures represent vitreous gel retained at the retinal surface, even though the structures could not be demonstrated by optical coherence tomography or indocyanine green staining at later vitrectomy surgery. This case should raise caution when considering vitrectomy surgery that requires posterior hyalold stripping in patients with good vision. [Ophthalmic Surg Lasers Imaging 2009;40:188-191.]
