Uyluğa ekstansiyon yaptırır, gövdeyi ayakta dik tutmada, yürümede ve merdiven çıkmada etkili bir kastır. Tamamının alınması durumunda kalça eklem fonksiyonlarının ve stabilitesinin bozulması kaçınılmazdır. Otuzsekiz yaşında erkek hastanın sağ gluteus maksimus kasının tamamı yumuşak doku sarkomu nedeniyle eksize edildi. Oluşan yumuşak doku defekti ve kalça eklem fonksiyonlarını onarmak için, saplı fonksiyonel anterolateral kas-deri flebi kullanıldı. Flep hazırlanırken, vastus lateralis kasının motor siniri fleple birlikte diseke edildi, böylece kasın fonksiyonel olması sağlandı. Bir buçuk yıldır takip edilen hasta herhangi bir zorluk yaşamadan yürüyebilmekte ve günlük aktivitelerini yerine getirebilmektedir. Saplı anterolateral uyluk flebi komplike kalça, uyluk, karın ve genital bölge defektlerinin tek aşamada fonksiyonel onarımı için uygun bir seçenektir.The gluteus maximus muscle keeps stability of hip and knee joints. It is a very effective muscle to provide extension of the hip joint, a good body posture, walking and climbing the stairs. Inevitably, if this muscle is removed completely, functions and stability of hips joint can be destroyed. The right gluteus maximus muscle of 38 year-old male patient was excised totally due to soft tissue sarcoma. For reconstruction of large soft tissue defect and to preserve hip joint functions, pedicled functional anterolateral musculocutaneous flap was used. During the flap harvest the motor nerve of the vastus lateralis muscle dissected with the flap to have a functional muscle. The patient, who has been followed for one and half year, can walk without any problem and he is active in daily life.
