Özet
Amaç: Vitiligolu hastalarda göz ve çevresini ilgilendiren bulguların araştırılması. Gereç ve Yöntem: Vitiligolu 50 hasta ve 60 kontrol hastasına ayrıntılı göz muayenesi yapılarak iki grup oftalmolojik bulgular yönünden karşılaştırıldı. Vitiligolu hastalarda kapak bulgularının diğer göz bulguları ile ilişkisi olup olmadığına bakıldı. Her iki grupta iridokorneal açıda pigmentasyon artışının yaşla ilişkisi değerlendirildi. İstatistiksel değerlendirmelerde ki-kare, Mann-Whitney U ve bağımsız gruplarda t testleri kullanıldı. Bulgular: İki grup arasında göz kapağında depigmentasyon (p=0.002) ve iridokorneal açıda pigment artışı (p=0.029) yönünden fark bulundu. Göz kapağında vitiligo lezyonu saptanan 9 hastanın 8’inde vücutta ve yüzde yaygın tutulum mevcut olup, lezyonların başlangıcından itibaren geçen süre 3-15 yıl arasında değişmekteydi. Vitiligolu hastalarda göz kapağı tutulumunun retinada iyi sınırlı depigmente alanlar (p=0.002) ve retina pigment epitel değişiklikleri (p=0.044) ile ilişkili olduğu görüldü. Her iki grupta da açıda pigment artışı olan hastaların yaş ortalamasının olmayanlara göre anlamlı derecede yüksek olduğu belirlendi (vitiligolu grup için p=0.009, kontrol grubu için 0.002). Sonuç: Özellikle göz çevresini de tutan ve uzun süreli yaygın tutulumu olan vitiligo hastalarında bazı göz bulgularının da olabileceği unutulmamalıdır. Hasta sayımız kesin yorumda bulunmak için düşük olsa da, vitiligolu hastalarda yapılan ayrıntılı oftalmolojik muayenenin bazı göz bulgularını ortaya çıkarmada faydalı olacağı görüşündeyiz.

The relationship of eyelid involvement with other ocular findings in vitiligo patients was investigated. The relationship between iridocorneal angle hyperpigmentation and age was evaluated in both groups. Chisquare, Mann-Whitney U, and t test for independent samples were used for statistical analyses. Results: There was a difference in eyelid depigmentation (p=0.002) and iridocorneal angle hyperpigmentation (p=0.029) between the two groups. There was diffuse involvement of the body and face in 8 of the 9 patients with vitiligo lesions in the eyelid and duration from the onset of the lesions changed between 3-15 years. Eyelid involvement in vitiligo patients was observed to be related with well-defined depigmented areas in retina (p=0.002) and retinal pigment epithelial changes (p=0.044). The mean age of patients with angle hyperpigmentation was significantly higher than the age of patients without hyperpigmentation in both groups (p=0.009, p= 0.002 for vitiligo and control groups, respectively). Conclusion: Ophthalmologists should be aware of the possible existence of some ophthalmological findings in vitiligo patients, particularly those with diffuse involvement and long duration lesions involving the periorbital region. Although the number of patients in our study is insufficient to make definite conclusions, we believe that detailed ophthalmological examination in vitiligo patients might be helpful in detecting some ocular findings
