Özet
Orbital leyomiyom, düz kaslardan köken alan, klinik, radyolojik ve histopatolojik olarak lezyonun bu bölgede nadir olarak görülmesi nedeniyle teşhisi zor olan benign bir tümördür. Otuz yaşında kadın hasta, sağ alt kapakta bir yıldır mevcut olan ağrısız şişlik şikayetiyle baş- vurdu. Sağ göz alt kapak nazalinde 9x8 mm boyutlarında, cilt altında hareketli, yumuşak kitle mevcut olup, eksizyonel biyopsi yapıldı. Histopatolojik ve immünohistokimyasal bulgulara da- yanılarak orbital leyomiyom tanısı konuldu. Orbital leyomiyom, orbitada herhangi bir lokali- zasyonda yerleşim gösteren, yavaş büyüyen bir tümördür. Posteriorda yerleşen tümörler da- mar duvarı düz kas hücrelerinden köken aldığı düşünülürken, anteriorda yerleşen tümörler ise kapsülopalpebral fasya veya Müller kasından köken alabilir. Radyolojik olarak orbitanın di- ğer benign lezyonlarından ayırt etmek için özgün bir bulgusu yokken, genellikle immünohisto- kimyasal analiz ile histopatolojik olarak tanı konulur.
Orbital leiomyoma is a benign tumour of smooth muscle origin that can be clinically, ra- diologically and histopathologically difficult to diagnose due to the rarity of leiomyomas in this location. A 30-year-old woman presented with complaint of painless right lower lid swelling for a year. There was a subcutaneous soft, mobile mass with a size of 9x8 mm on the nasal side of right lower eyelid and excisional biopsy was performed. On the basis of the histopathological and immunohistochemical findings, the diagnosis of orbital leiomyoma was established. Orbital leiomyoma is a slow-growing tumor that can be located anywhere in the orbit. Posterior tumors are believed to originate from the smooth muscle cells of vessel walls; anterior tumors may arise from the capsulopalpebral fascia or Müller muscle. Although there are no unique features that help radiologically to exclude other benign lesions of the orbit, the histopathologic diagnosis using immunohistochemical analysis is usually straightforward.
