Öğe The effect of radiofrequency ablation treatment on quality of life and anxiety in patients with supraventricular tachycardia(Verduci Publisher, 2012) Yıldırım, Osman; Yontar, Osman Can; Semiz, M.; Erdem, Alim; Canan, F.AIM: Paroxysmal supraventricular tachycardia (PSVT) has been shown to be associated with poor outcome in psychological status and quality of life. This study aimed to assess the quality-of-life and anxiety in patients with PSVT and to investigate the influence of radiofrequency ablation (RFA) treatment on these parameters. MATERIALS AND METHODS: Fifty patients with newly diagnosed PSVT with electrophysiologic study and treated with RFA, were enrolled in the study and 50 healthy age-and sex-matched individuals. The brief version of World Health Organization Quality of Life Scale and the state-trait anxiety inventory were administered to controls and all patients before and three months after the RFA procedure. RESULTS: At baseline, in comparison with the control group, the patients with PSVT exhibited greater anxiety average scores (p < 0.05). After RFA procedure, the patients with PSVT exhibited significant improvement in anxiety, quality of life and health satisfaction scores. Statistically significant difference between the controls and the patients in respect to state and trait anxiety average scores disappeared after treatment (p > 0.05). CONCLUSIONS: In this study, normalization of anxiety and quality of life is associated with RFA treatment for patients with PSVT. This finding need to be confirmed by larger scale clinical trials with long-term quality of life follow-up in higher number of patients.Öğe The effect of smoking on the baroregulatory system; heart rate turbulence(2012) Erdem, Alim; Yalta, Kenan; Yilmaz, Mehmet Birhan; Turgut, Okan Onur; Yilmaz, Ahmet; Özkök, Mesut; Yontar, Osman CanGiriş: Sigaranın kardiyovasküler sisteme olan zararlı etkileri bilinmektedir. Çalışmamızın amacı sigara içiminin baroregülatuvar mekanizmaya olan etkilerini, kalp hızı türbülansını kullanarak normal kişilerle karşılaştırmaktır. Materyal ve Metod: En az bir yıldır sigara içme öyküsü olan asemptomatik 64 kişi grup 1 olarak kabul edildi. Aynı klinik özelliklere sahip asemptomatik sigara alışkanlığı olmayan 30 kişi grup 2 olarak düzenlendi. Çalışmaya alınan grup 1’deki sigara içicilerinin nikotin bağımlılık indeksi (NBİ) modifiye-Fagerström Nikotin bağımlılık testi (m-FNBT) kullanılarak hesaplandı. Tüm gruplardaki hastalara 24 saatlik elektrokardiyografi Holter takılarak, tamamının HRT (TO, total onset ve TS, total slope) ile bazal ekokardiyografik parametreleri hesaplandı. Bulgular: Gruplar arasında taşıdıkları demografik, klinik ve ekokardiyografik özellikler açısından istatistiksel olarak anlamlı bir fark saptanmadı (p> 0.05). Grup 1’de ortalama TO değeri anlamlı olarak yüksek iken (p< 0.05), ortalama TS değerleri arasında iki grup arasında istatistiksel olarak anlamlı bir fark saptanmadı (p> 0.05). Grup 1’de hesaplanan NBİ değeri ile ortalama TO arasında pozitif korelasyon saptandı (p< 0.05). Sonuç: Kronik sigara içicisi olanlarda, aynı özelliklere sahip içmeyen gruba oranla baroregülatuvar mekanizmada (özellikle TO) bozulma olduğu saptandı.Öğe The pure effects of obstructive sleep apnea syndrome on cardiac autonomic functions: heart rate turbulence analysis(Verduci Publisher, 2013) Erdem, Alim; Doğan, Ömer Tamer; Yontar, Osman Can; Epöztürk, Kürşat; Özlü, Mehmet Fatih; Öztürk Serkan; Ayhan, Suzi Selim; Yazıcı, Mehmet; Talay, FahrettinOBJECTIVES: To demonstrate the pure effect of obstructive sleep apnea syndrome (OSAS) on cardiac autonomic function (CAF) using heart rate turbulence (HRT) parameters. PATIENTS AND METHODS: A total of 64 patients with OSAS and 30 age-and gender-matched healthy subjects were enrolled. All subjects had normal coronary arteries and were free from diabetes mellitus (DM) and hypertension (HT). The HRT parameters (TO, turbulence onset; TS, turbulence slope) were obtained from 24-h ambulatory electrocardiogram (ECG) recordings. HRT parameters were compared between groups, and the relationship between HRT and the apneahypopnea index (AHI) was examined. RESULTS: No between-group differences were found in age or gender. Mean TO was significantly higher in the OSAS group than in healthy controls (0.89 +/- 0.5, -0.08 +/- 0.26; p < 0.001; respectively). The mean TS did not differ between the two groups (2.81 +/- 3.06 versus 3.14 +/- 2.33; p = 0.212). The AHI was positively correlated with TO (r = 0.845, p < 0.001). The multiple logistic regression analysis revealed that after adjustment for other variables, TO was a significant and independent predictor of AHI, OR 2.394 (95% CI: 1.596-3.591). CONCLUSIONS: HRT (TO in particular) is correlated with AHI. Thus, impaired HRT may be an important factor underlying the occurrence of arrhythmia and sudden cardiac death in patients with OSAS.Öğe Relationship between metabolic syndrome and non-dipping blood pressure pattern in obese patient(2013) Yılmaz, Ahmet; Erdem, Alim; Karapınar, Hekim; Küçükdurmaz, Zekeriya; Gül, İbrahim; Yontar, Osman CanAmaç: Metabolik sendrom ve non -dipper kan basıncı paterni kardiyovasküler hastalıklarla ilişkilidir. Bu klinik durumların ortak patofizyolojik mekanizmalarıyla ilişkili olabilir. Metab olik sendrom ile non -dipper kan basıncı arasındaki ilişkiyi araştırmayı amaçladık. Yöntem: 24 saatlik ambulatuar kan basıncı takibi yapılan yeni tanı almış 118 obez metabolik sendromlu hasta çalışmamıza dahil edildi. Bunlar dipper (n = 46) ve non -dipper (n = 72) olmak üzere iki gruba ayrıldı. Gece non -dipper kan basıncı, gece ortalama sistolik ve/veya diastolic kan basıncı azalması %10 dan daha az azalma oluşu olarak tanımlandı. Bulgular: Ça lışma gurubumuzun ortalama 24 saatlik kan basıncı ortalaması 146/96 mmHg idi. İki grubun temel karakteristik özellikleri benzer olmasına rağmen, non -dipper tansiyon paterni olan grupta metabolik sendrom sıklığı a nlamlı derecede daha fazla idi (p=0.038). Sonuç: Bu çalışmada non -dipper kan basıncı paterni olan hastalarda metabolik sendromun daha fazla bulunduğu saptandı . Bu durum, obez hastalarda metabolik sendrom ile non-dipper kan basıncının ortak bir patofizyolojik mekanizma içerdiğini belirtebilir.Öğe A relationship between mitral valve prolapse and subtypes of supraventricular tachycardia(Springer, 2012) Erdem, Alim; Yontar, Osman Can; Öztürk, Serkan; Ayhan, Suzi Selim; Özlü, Mehmet Fatih; Erdem, Fatma Hizal; Yazıcı, MehmetThe aim of this study was to investigate the frequency of mitral valve prolapse (MVP) in patients with supraventricular tachycardia (SVT) who were diagnosed by intracardiac electrophysiological study (EPS). The study was a retrospective cohort of 378 adult patients (128 males and 250 females, 17 to 63 years) admitted with EPS. We included 267 SVT patients (58.4 % (n = 156) atria-ventricular nodal reentrant tachycardia (AVNRT), 30 % (n = 80) atria-ventricular reentrant tachycardia (AVRT), 6.4 % (n = 17) atrial tachycardia (AT), and 5.2 % (n = 14) other). MVP prevalence was compared in individuals with SVT. MVP prevalence was found 27.7 % (n = 74) in patients with SVT. In subgroup analyses, there was a significant difference between AVRT and others about MVP prevalence (AVRT 52.7 % n = 39, AVNRT 33.8 % n = 25, AT 9.5 % n = 7, and others 4.1 % n = 3; p < 0.005). Also, there was a significant difference between females and males (75.7 % (n = 56); 24.3 % (n = 18); p < 0.05). There was a significant negative correlation between age and MVP just in female group (r = -0.116, p = 0.024). It was demonstrated in our study that AVRT and female sex may be related to MVP in SVT patients. It was dedicated from these results that the existence of arrhythmia should be focused on young female subjects with MVP more than the others.
