    Association of extent and aggressiveness of inflammation with serum PSA levels and PSA density in asymptomatic patients

    OBJECTIVES The relationship between serum prostate-specific antigen (PSA) levels and histologic prostatic inflammation is controversial. Previous studies were performed using either the intensity or extent of inflammation for grading, with a relatively lower number of specimens. In our study, the inflammation was categorized more profoundly, using both the intensity and the extent of inflammation, to determine the influence of prostatic inflammation on serum PSA levels, percent free PSA (%fPSA), and PSA density (PSAD). METHODS The study included 115 patients who underwent transrectal ultrasound-guided prostate biopsy. To categorize the inflammation, a grading method that included the intensity and extent of inflammation was used. The extent and aggressiveness of inflammation were analyzed. The patients were divided into groups using five grades for the extent and four grades for the aggressiveness of inflammation. The serum PSA Levels, fPSA levels, %fPSA, and PSAD in each group were compared. RESULTS The extent of inflammation grade correlated positively with the serum PSA level (r = 0.423, P <0.001) and PSAD (r = 0.319, P = 0.001). However, a negative correlation was found between the extent of inflammation grade and %fPSA (r = -0.268, P = 0.015). The aggressiveness of inflammation grade correlated positively with the serum PSA level (r = 0.386, P <0.001) and PSAD (r = 0.341, P = 0.001) and negatively with %fPSA (r = -0.289, P = 0.03). CONCLUSIONS If the elevation of serum PSA is thought to be caused by histologic inflammation, it might prevent unnecessary repeated biopsies.

    Does diagnostic sonography alter thyroid and parathyroid hormone levels?

    Purpose. To investigate possible alterations in the levels of thyroid and parathyroid hormones (PTHs) and thyroglobulin (TG) in healthy individuals following diagnostic sonographic examination of the thyroid gland. Methods. Thirty healthy women with no pathologic findings underwent sonographic examination, followed 6 weeks later by a second examination involving a probe-only application (PA) with the ultrasound scanner switched off. Duration times were identical for both examinations. Blood was drawn before and after the 2 applications. Thyroid hormone, PTH, and TG levels before and after the 2 applications were compared, and the difference between the variations for each parameter in the first and second applications was assessed. Blood samples were taken before and after the sonographic examination and the PA, and the serum concentrations of sensitive thyrotropin, total and free thyroxine, total and free tri-iodothyronine, TG, and PTH were measured. The pre- and post-examination levels of the hormones for the 2 applications were then compared and the difference between the variations for each parameter in the first and second application was then assessed. Results. The only significant variations observed were in the TG levels after PA and PTH levels after both sonographic examination and PA. The comparison between the 2 difference values revealed no significant difference except for PTH. Conclusion. This preliminary report on the possible influence of sonographic examination of the thyroid on the serum levels of thyroid and parathyroid hormones suggests that gland secretions such as PTH may be affected by external factors, including ultrasound. Clinicians should be aware of alterations in hormone levels by external factors. (c) 2007 Wiley Periodicals, Inc.

    Effect of beta-glucan in lung damage secondary to experimental obstructive jaundice

    Background/aims: This study aimed at investigating the protective effects of beta-glucans on the lungs in obstructive jaundice. Methods: In total, five groups -Sham (1), control (2) and treatment groups (3,4,5)- were established; each comprising randomly selected seven Wistar Albino rats. Beta-glucan was given after choleduct ligation in Group 3 while it was given before and after the choleduct ligation in Group 4. As pre-treatment beta-glucan was given before ligation in Group 5. Beta-glucan was administered in a single dose of 50 mg / kg / day by gavage for a ten-day period. Superoxide dismutase, and myeloperoxidase levels in serum; malotzdialdehyde, lipid hydroxyperoxidase and glutathione levels in lung tissue; lactate dehydrogenase levels in bronchoalveolar lavage fluid were measured. Results: The blood polymorphonuclear leukocytes level was highest in the control group and lower in the sham and treatment groups. Serum superoxide dismutase and tissue glutathione values were significantly higher in Groups 3 and 4 (p <= 0.04) whilst Groups 3 and 4 did not differ from each other. In Groups 3 and 4 malondialdehyde, lipid hydroxyperoxidase, and myeloperoxidase values were significantly lower. However, Groups 3 and 4 did not differ for malondialdehyde or lipid hydroxyperoxidase values. Lactate dehydrogenase level in the bronchoalveolar lavage fluid was significantly lower in all of the treatment groups (Groups 3,4,5) (p <= 0.008). When compared to the control group, it was observed that lung damage was much more limited in the treatment groups (p<0.001). Conclusion: This study suggests that beta-glucan exhibits protective effect in pulmonary tissue against oxidative damage in obstructive jaundice.

    Effect of epidural levobupivacaine and levobupivacaine with fentanyl on stress response and postoperative analgesia after total knee replacement

    Background: Providing sufficient and convenient analgesia is crucial during the postoperative period after total-knee replacement (TKR) to enhance patient mobility and reduce stress response to surgery. The scope of this study is to compare the effects of levobupivacaine and levobupivacaine plus fentanyl on stress response and analgesic efficiency after TKR. Method: In this study, 40 ASA I - II patients scheduled to undergo TKR were subjected to combined spinal epidural anesthesia (CSEA) injecting of 15 mg levobupivacaine and randomly assigned to receive either levobupivacaine 0.125% (Group L) or levobupivacaine 0.125% plus fentanyl 4 mu g ml(-1) (Group F) during postoperative period via the epidural route. Patient controlled epidural analgesia (PCEA) was offered for 24 hours. Venous blood samples were assayed for ach-enocorticotropic hormone (ACTH), cortisol and prolactin levels before surgery and after analgesia administration. Analgesia was assessed using a visual analogue scale (VAS) at rest (VASR) and during movement (VASM). Results: There was no statistically significant difference between the groups in terms of total doses, bolus requests, bolus delivered and side effects (p > 0.05). The ACTH, cortisol and prolactin levels increased following the surgery and decreased during PCA infusion in both groups where the decline in Group F was significant (p < 0.05) at 24 hours after the analgesic treatment and 48 hours after the surgery. Conclusion: We have demonstrated that infusion of levobupivacaine (0.125%) in combination with fentanyl (4 jig ml-1) using PCEA suppressed stress response to surgery significantly and provided better pain relief than levobupivacaine (0.125%) alone after TKR.

    Effect of manual lymph drainage on removal of blood lactate after submaximal exercise

    [Purpose] It has been well-established that exercise-induced muscle damage occurs following intense exercise. Massage is commonly used to manage muscle damage resulting from exercise. However the effect of massage after exercise is still not clear. The purpose of this study was to examine the effect of manual lymph drainage on muscle damage and on the removal of blood lactate following submaximal exercise (SE), as part of a solution to the challenging problem in sports medicine of muscular recovery after exercise. [Subjects and Methods] Eighteen healthy male students, with moderate exercise training, were randomly assigned to either receive manual lymph drainage (MLD) or serve as controls. Both groups were subjected to a graded exercise test, performed on a treadmill ergometer, to determine each subject's individual anaerobic threshold (IAT). Seven days later, all subjects were made to run for 30 minutes on the same treadmill ergometer, at a running speed equivalent to the IAT. One group received MLD treatment, while the control subjects received no treatment. [Results] Following an increase immediately after exercise, lactic acid (LA) and lactate dehydrogenase (LDH) serum levels dropped rapidly and significantly at the end of MLD application and two hours after SE in the subjects receiving MLD. The course of creatine kinase (CK) and myoglobin levels was comparable, and with myoglobin showing a significant difference at 2 h after SE, and CK at 24 h after SE. [Conclusion] Manual lymph drainage after SE correlated with a more rapid fall in LA and of the muscular enzymes of LDH, CK and myoglobin, and may have resulted in an improvement in the regenerative processes elicted by structural damage to the muscle cells.

    The effect of melatonin on tinnitus with respect to sleep and depression : a randomized clinical trial

    Objective: In this study we aimed to investigate the psychological status of tinnitus patients as well as the sleep quality and their relation to handicap caused by tinnitus using various scales. Additionally, serum melatonin levels and the effectiveness of melatonin treatment were studied. Study Design: Prospective, double blind, randomized controlled trial Materials and Methods: Patients were divided randomly into two groups as study (melatonin, n=13) and placebo (control, n=11) groups. Tinnitus Handicap Inventory (THI), Symptom Check List (SCL), Hospital Anxiety and Depression Scale (HAD), Beck Depression Inventory (BDI), Pittsburg Sleep Qualty Index (PSQI) were applied. After filling the scales and giving the venous blood sample for melatonin measurements the patients were instructed to take one tablet (placebo or 3 mg melatonin) before sleep every night. After 8 weeks, second order scales were filled and melatonin measurements were repeated. Results: Serum melatonin levels were between 1-260 pg/ml; the mean was 38,7 pg/ml. The correlation of melatonin levels with THI and tinnitus duration was not significant. THI was found to correlate with different measures of the PSQI, HAD, and BDI in both groups. Statistical analysis failed to show any significant difference within and between groups in respect of anxiety, depressive symptoms and sleep as well as melatonin and handicap levels. When the groups were assessed according to the THI severity (mild/moderate to severe;THI2); in the control group there was significant differences in PSQI(1) and PSQI(1) (p=0.0008, p=0.18), NAD(1), HAD(2) (0.002, 0.03), HAD Depression(1-2) (0.0, 0.006) BDI (p=0.007) PSQI(2 sleep) (disturbance) (P=0.018) parameters. However, in the melatonin group it was found that there were significant differences in SCL2 (sleep) (latency), PSQI(2) (sleep) (duration) and total PSQI(2) parameters. (p=0.022, 0.027, 0.006 respectively) Conclusion: Patients with higher handicap may benefit melatonin in respect of sleep latency and duration as well as sleep quality comparing with the patients taking placebo. Moreover, melatonin efficiency may be related to its antidepressive effect.

    The effect of pinealectomy and leptin hormone on the proliferation and apoptosis activation in Syrian hamster testis in different photoperiods

    P>The effects of pinealectomy and leptin hormone on proliferative and apoptotic processes in the epithelia of testicular seminiferous tubules of Syrian hamsters have been investigated. Proliferative and apoptotic processes were assessed semi-quantitatively by proliferating cell nuclear antigen (PCNA) and caspase-3 immune stainings. Animals used in the study were divided into four groups; control, pinealectomy (PinX), leptin-treated (10 mu g/mL/day/kg body weight, intraperitoneally) and pinealectomy + leptin groups. Half of the hamsters in each group were exposed to short and the other half to long photoperiods for 8 weeks. In short photoperiod, PCNA activity especially in spermatogonia was significantly higher in the pinealectomy and leptin-treated groups compared with the control group. Histological score (HSCORE) value of PCNA in the PinX + leptin group was lower than those of PinX and leptin-treated groups. HSCORE value of caspase-3 in PinX and PinX + leptin groups was increased. In the long photoperiod, PCNA activation in the PinX group was significantly lower than the control group while the differences between the controls and other groups were not significant. The difference between the increases in caspase-3 activity in the PinX and control groups was significant. Thus, it was observed that photoperiods had no effect on the proliferation activity in the control groups. The inhibiting effect of short photoperiod on testis was not observed throughout 8 weeks. PinX eliminated the inhibiting effect of short photoperiod but did not alter the stimulating effect of long photoperiod. Leptin did not show any effect in long photoperiod but decreased proliferation by stimulating melatonin in short photoperiod.

    Effect of tube filling order on specific coagulation parameters in healthy subjects

    Abstract Background: In this study, the authors attempted to determine if there was any effect of tube filling order on both routine (prothrombin time [PT], activated partial thromboplastin time [APTT], and fibrinogen) and specific coagulation parameters (protein S, protein C, antithrombin III, factor V Leiden, plasminogen, and D-dimer). Methods: Fifty-seven consecutive patients having coagulation parameters within the reference range (mean age ±SD, 32.14±5.59 years) were included in the study. In drawing blood, the first 2 tubes were 3.2% citrated coagulation tubes. Routine and specific coagulation parameters were studied on both tubes, and the results were compared using a paired sample t-test. Results: The comparison of all parameters revealed statistically-insignificant results. The difference in the levels of protein C between the 2 tubes approached significance (P=0.053). Conclusion: This study has shown some evidence that tube filling order in healthy subjects does not play a significant role on routine and specific coagulation parameters.

    The effects of mistletoe (Viscum Album L. Subsp. Album) extracts on isolated intestinal contractions

    Throughout history folk medicine has served as an alternative or complimentary method of treating disorders like intestinal colic. Mistletoe is a common name for many species of semi-parasitic plants that grow on trees throughout the world, and has been used widely to treat intestinal colic. We conducted a study to evaluate the effect of freeze-dried (group 1) and heat-treated (group 2) extracts of Viscum album subsp. album leaves on contractions induced by acetylcholine in isolated hamster intestinal segments (duodenum, jejunum, and ileum). Freeze-dried extracts decreased intestinal contractions in all the intestinal segments (P < 0.05). The strongest relaxant effect was observed in the jejunum and the weakest was seen in the duodenum. Higher doses of heat-treated extracts were required to affect segmental contractions than that of freeze-dried extracts (P < 0.05); heat-treated extracts had the greatest effect on reducing contractions in the jejunum and the least effect in the duodenum. Mistletoe extracts affected the level of contractions in a dose- and extraction preparation-dependent manner. The results of this study show that mistletoe extracts could be effective against intestinal motility problems and that they have potential use as a therapeutic remedy for intestinal colic.

    Halk ilaçları eski çağlardan bu yana bağırsak ağrıları gibi problemlerin tedavi edilmesinde alternatif ya da tamamlayıcı bir yol olmuşlardır. Ökseotu, tüm dünyada, ağaçlarda büyüyen gelişen yarı parazit bir bitkidir ve bağırsak kasılmalarına karşı yaygın olarak kullanılmaktadır. Çalışmamızda Viscum album subsp. album yapraklarının, bitkisinden dondurulup-kurutulma (Grup 1) ve sıcak su (Grup 2) ile ekstraksiyon suretiyle hazırlanan ekstraktların, izole edilmiş ince bağırsağın farklı segmentlerinde (duodenum, jejunum, ileum) asetilkolin ile oluşturulmuş kasılmalar üzerine olan etkilerini inceledik. 1. Gruptaki ekstraktlar tüm bağırsak segmentlerinde kasılmaları düşürdü (P < 0.05). En güçlü gevşetici etki jejunumda, en düşük etki ise duodenumda gözlendi. 2. Gruptaki ekstraktlar 1. Gruptaki ekstraktlardan daha yüksek dozlarda segmental kasılmaları etkiledi (P < 0.05); kasılmalardaki düşüş jejunumda daha fazla, daha düşük ileum ve en düşük etki duodenumda gözlendi. Ökseotu ekstraktlarının kasılmaları doza ve hazırlama yöntemlerine bağlı olarak etkilediği görülmektedir. Sonuç olarak ökseotu ekstraktları bağırsak hareket problemlerine karşı etkili olabilmekte ve bağırsak kasılmalarında tedavi edici ilaç olarak bir potansiyeli bulunmaktadır.

    Effects of photoperiod and food availability on growth, leptin, sexual maturation and maintenance in the Mongolian gerbils (Meriones unguiculatus)

    Reproductive activity of Mongolian gerbils is regulated by photoperiod nevertheless body weight regulation is controlled without ambient photoperiod. Food intake is a major factor affecting rodent reproductive efficiency. Leptin is a hormone secreted by adipose tissue and modulates food intake, energy expenditure and body fat stores. In this study we studied the interaction of photoperiod and food availability on growth, sexual maturation and leptin concentration in the male and female gerbils. Gerbils were gestated and reared in either 14L:10D or 2L:22D. At weaning, gerbils were housed individually and divided into three groups: fed ad libitum, fed 80% of ad libitum or fed 60% of ad libitum. Body weights were recorded at weaning and every week thereafter. After twelve weeks of treatment, animals were sacrificed and testes and uterine weights were determined and blood was collected for leptin measurement. Food restriction reduced body weight and inhibited reproductive development. Absolute paired testis weights were similar in ad lib and 80% of ad lib groups but significantly different compared with the 60% of ad lib group in both photoperiods. Body weights were also directly dependent upon the level of food restriction. Uterine mass was only affected in the 60% of ad lib group in 14L but both food restriction levels significantly affected the uterine weights in 2L. Significant variations were found in leptin profiles. Leptin concentration was highest in ad lib and 80% of ad lib groups and lowest in 60% of ad lib groups. These results suggest that the reproductive activity of Mongolian gerbils is sensitive to food intake and multiple potential environmental cues (e.g., food availability, temperature) can be utilized.

    The effects of physical therapeutic agents on serum levels of stress hormones in patients with osteoarthritis

    To investigate the effects of physical agents on the levels of stress hormones in patients with osteoarthritis (OA).Transcutaneous electrical nerve stimulation, hot packs, and therapeutic ultrasound were applied to the lumbar region and knees of patients with OA. Blood samples were taken for the measurement of the serum levels of glucose, insulin (INS), growth hormone (GH), prolactin (PRL), cortisol (COR), and plasma adrenocorticotropic hormone (ACTH) immediately before and after the 1st session, to investigate the acute effects of those physical agents on the endocrine system. The hormone levels were also measured every 5 sessions in a total of 10 sessions. The treatment response was also evaluated by using the visual analogue scale (VAS), Roland Morris Disability Questionnaire (RMDQ), and Western Ontario and McMaster Universities Arthritis Index (WOMAC) throughout the therapy period.After the 1st session, there was a decrease in INS levels and a mild decrease in PRL levels (P=0.001 and P<0.05, respectively). Throughout the 10-session therapy period, the INS levels increased, whereas the ACTH and COR levels decreased (P<0.05 for all). The VAS-spine, RMDQ, VAS-knee, and WOMAC scores decreased (P=0.001 for VAS-spine and P<0.001 for all others). A positive correlation was detected between the changes in serum COR and WOMAC-pain score (P<0.05).Although the combination therapy caused changes in INS level accompanied with steady glucose levels, the application of physical agents did not adversely affect the hormone levels. The decrease in ACTH and COR levels may be attributed to the analgesic effect of agents and may be an indicator of patient comfort through a central action.

Gereç ve Yöntemler: Laboratuvarımıza kan vermek üzere başvuran, paratiroid patolojisi bulunmayan 50 hastadan ardarda 4 tüpe (2 EDTA'lı, 2 jelli tüp) kan alınmış ve bunlardan EDTA'lı ve jelli tüplerden birer adedi soğukta (buza gömülerek), birer adedi de oda ısısında olacak şekilde santrifüjlemeye kadar 15 dk. bekletilmiştir. Daha sonra soğukta bekletilenler +4˚C'de, oda ısısında bekletilenlerse normal santrifüjle çevrilerek serum ve plazmalar ayrılmıştır. Tüm örneklerin PTH düzeyleri/Immülite 2000 hormon analizöründe aynı gün çalışıldı. PTH değerleri referans aralığının (16-87 pg/mL) dışında kalanlar çalışmaya dahil edilmemiş, böylece geriye kalan 30 kişinin örnekleri istatistiksel olarak değerlendirilmiştir. Normal serum-soğuk serum, normal plazma-soğuk plazma, normal serum-normal plazma ve soğuk serum-soğuk plazma şeklinde yapılan eşleştirmelerin istatistiksel değerlendirmesi için eşleştirilmiş t-testi kullanılmıştır. Bulgular: Tüm eşleştirmeler arasında istatistiksel olarak anlamlı fark bulunmuştur (sırasıyla p= 0.035, p< 0.001, p< 0.001, p< 0.001). PTH değerleri EDTA'lı plazmalarda seruma göre daha yüksek saptanmıştır. Bu durum soğukta biraz daha yüksek, oda ısısında biraz daha düşük sonuçlarla birliktelik göstermiştir. Sonuç: Kan alma noktalarında PTH ölçümleri için soğuk zincire uyularak kan alınıyorsa veya serum yerine EDTA'lı plazmadan çalışılıyorsa, bu durumun kliniklerden gönderilen kanlar için de standart hale getirilmesi, yatan hasta ve ayaktan hastalar arasında gözlenebilecek PTH sonuçları uyumsuzluğunu ortadan kaldırabilir. Objective: The purpose of this study was to investigate parathyroid hormone (PTH) measurements in two types of matrices (serum and plasma) collected and kept under room temperature and cold conditions. Material and Methods: Blood was drawn successively into 4 tubes (2 with EDTA, 2 with clot-activator gel) from 50 patients without parathyroid pathology who attended to our laboratory. Two of the tubes (one with EDTA and one with gel) were kept in cold (embedded in ice) and the other two in room temperature for 15 minutes until centrifugation. Then, the tubes kept in cold were centrifuged at 4 degrees C and the tubes kept in room temperature underwent normal centrifugation to separate the sera and plasma. PTH levels were measured in all sample groups with Immulite 2000 immunoassay on the same day of blood collection. PTH levels outside the reference limits (16-87 pg/mL) were not included in the study and the samples from the remaining 30 subjects were statistically evaluated. Paired t-test was used for evaluating the statistical difference between the following pairs: normal serum-cold serum, normal plasma-cold plasma, normal serum-normal plasma, cold serum-cold plasma. Results: The difference between all pairs were statistically significant.(p= 0.035, p< 0.001, p< 0.001, p< 0.001, respectively). PTH levels were higher in plasma with EDTA than in sera. This was associated with relatively higher levels in cold and lower levels in room temperature. Conclusion: If blood is drawn according to cold chain rules for PTH measurements at blood drawing units or plasma with EDTA is used instead of serum, standardization of this process for the blood sent from the clinics can eliminate the disparity in PTH measurements between outpatients and inpatients.

    Food restriction affects locomotor activity in Mongolian Gerbils (Meriones Unguiculatus)

    The circadian rhythm of the locomotor activity is regulated by at least 2 external pacemakers, environmental photoperiod and food availability. In the present study, the effects of short-term (2 weeks) food restriction on locomotor activity rhythm in adult male and female Mongolian gerbils were investigated. Animals were fed ad libitum with no restriction during the first week of the experiment and their daily ad libitum food consumptions were determined. Then the food was restricted 40% of ad libitum for the next 2 weeks. At constant temperature, locomotor activity was monitored by running wheel in animals exposed to a long photoperiod (14L) for 3 weeks. A significant phase advance was recorded in the locomotor activity rhythm in both male and female animals. The shift was bigger in males (12 ± 1.1 h) than in females (6 ± 0.5 h) (P < 0.05). Activity levels and body weights decreased in males (P < 0.01) and females (P < 0.012) compared with the first week values. The gerbils' locomotor activity was significantly affected by the food restriction. These results indicate that the rhythm and the level of the locomotor activity in the Mongolian gerbil can be entrained by food restriction.
