Öğe Amount of lateral cortex loss in the femur while inserting a DHS-plate(Tubitak Scientific & Technical Research Council Turkey, 2013) Aytekin, Mahmut Nedim; Küçükdurmaz, Fatih; Esmer, Ali Fırat; Sağlam, Necdet; Akpınar, FuatAim: Although many anatomic, morphologic, and radiologic studies have been carried out for the femur as for all other bones of the skeletal system, we could not find any morphometric study to guide orthopedic surgeons on dynamic hip screw-plate (DHS-plate) applications of the proximal femur. Materials and methods: The lateral cortex width of 125 adult, dry, and grossly intact right and left cadaver femora was measured for the region of the femur in which the DHS-plate was applied, and the results were analyzed statistically. Results: The mean value of the proximal lateral cortex width of the femora was 27.63 mm. While inserting the DHS-plate, 45.24% of the mean width of the lateral cortex was lost. Conclusion: In this study we demonstrated the amount of lateral cortex loss in the femur while inserting a DHS-plate. New mechanical studies may be needed to show the amount of lateral cortex loss that leads to the fracture.Öğe Biomechanical comparison of antirotator compression hip screw and cannulated screw fixations in the femoral neck fractures(2014) Sağlam, Necdet; Küçükdurmaz, Fatih; Kıvılcım, Hakan; Kurtulmuş, Tuhan; Şen, Cengiz; Akpınar, FuatAmaç: Bu çalışmanın amacı, Pauwels tip 3 femur boyun kırık modelinde, minimal invaziv kayıcı antirotator kompresyonlu kalça vidası (MIS–A–CHS) ile 3 kanüle vidanın (KV) tespit gücünü biyomekanik açıdan karşılaştırmaktı. Çalışma planı: On iki adet üçüncü nesil proksimal femur kemik modelinde Pauwels tip 3 vertikal femur boyun kırığı oluşturuldu ve modeller iki gruba ayrıldı. Birinci gruba üç paralel KV, ikinci gruba MIS–A–CHS ile tespit uygulandı. İki grubun aksiyel ve torsiyonel yüklenmelere karşı dayanıklılıkları ve maksimal kompresyon güçleri hesaplandı ve birbirleri ile karşılaştırıldı. Bulgular: MIS–A–CHS grubundaki maksimal aksiyel yüklenme sınırı (912.5 N), maksimal torsiyonel yüklenme sınırı (540 N) ve maksimal kompresyon gücü (10.2 N/m); KV grubundaki değerlerden (sırasıyla 627.5 N; 380 N ve 3.9 N/m) anlamlı olarak yüksek bulundu (p<0.05). Çıkarımlar: MIS–A–CHS, Pauwels tip 3 femur boyun kırıklarında KV’e göre aksiyel ve torsiyonel kuvvetlere daha dayanıklı olan ve daha fazla kompresyon sağlayan bir tespit yöntemidir.Öğe Bioremediation applications with fungi(Springer International Publishing Ag, 2018) Sağlam, Necdet; Yeşilada, Özfer; Sağlam, Semran; Apohan, Elif; Sam, Mesut; İlk, Sedef; Emul, Ezgi; Gürel, EkremIndustrial wastewaters such as olive oil mill wastewater (OOMW) and alcohol factory wastewater (vinasse) with high polluting characteristics affect the ecosystem seriously. The environmental impact of these wastewaters is rather severe due to their organic matter content and dark color. Therefore, it is illegal to dispose these wastewaters directly into the environment. In addition of wastewater treatments, decolorization of Kraft mills is one of the serious problems in pulp and paper industries because of their high color contents. The discharges of Kraft mills present a threat for environment and especially surface waters. These colored effluents contain serious amounts of chlorinated and oxidized lignin compounds. Remediation of this kind of wastewaters by conventional treatment methods has difficult and challenging processes. As a solution, the fungi have been chosen for bioremediation of wastewaters as efficient biological systems as they are able to remove the color and threatful organic content. Mycoremediation is an effective and ecofriendly method for the bioremediation of this type of wastewaters. This method has several advantages over chemical or physical remediation. Besides, fungal enzymes have a great potential for detoxifying and screening most of the environmental pollutants. In the last decade, fungal enzymes have a new application area on sensor technology. Biosensors are able to utilize fungal enzymes including an electrode that may be used for the detection of pollutants such as phenolic compounds. The objective of this chapter is to summarize recent developments in mycoremediation of wastewaters, especially OOMW and vinasse. Here, the role of biotechnologically important fungi such as yeast, molds, and white rot fungi in the mycoremediation of wastewaters is reviewed. The various mycoremediation methods for effective bioremediation and recent developments for fungal enzymes are also discussed.Öğe Distal phalanx fracture in adults : Seymour-type fracture(Sage Publications Ltd, 2014) Uğurlar, Meriç; Saka, Gürsel; Sağlam, Necdet; Milcan, Aptullah; Kurtulmuş, Tuhan; Akpınar, FuatSeymour's fracture is an extra-articular, transverse, physeal, and juxta-epiphyseal open fracture of the distal phalanx seen in childhood. In this study, we present 10 adult cases of fractures localized to the metaphyseal region, 1-2 mm distal to the insertion of the extensor tendon. Mean age was 38 years. Four patients were treated conservatively with a mallet finger splint. Closed reduction and osteosynthesis with Kirschner wires (K-wires) was performed in three of the patients. Three of the patients had open fractures in whom closed reduction could not be performed. They underwent open reduction and osteosynthesis with K-wires. We recommend that extra-articular distal phalanx fractures mimicking mallet finger in adults are called Seymour-type fracture to establish a common language among clinicians to define this type of fracture. These fractures generally occur by hyperflexion of the distal phalanx and can be treated by conservative or surgical methods. The outcomes of conservative and surgical management of Seymour-type fractures depend on the appropriate reduction as well as efficient physical therapy.Öğe Innovation of strategies and challenges for fungal nanobiotechnology(Springer International Publishing Ag, 2016) Sağlam, Necdet; Yeşilada, Özfer; Çabuk, Ahmet; Şam, Mesut; Sağlam, Semran; Gürel, EkremNanotechnology involves the study and use of materials under the 100 nm scale, exploiting the different physiochemical properties exhibited by these materials at the nanoscale level. Promising applications have been already employed in the areas of drug delivery systems using bioactive nanoencapsulation, biosensors to detect and quantify pathogens, chemical and organic compounds, alteration of food compositions, and high-performance sensors and film to preserve fruits and vegetables. Moreover, the taste of food and food safety can be improved by new nano-materials from the microbiological sources. The huge benefits from this technology have led to increases in the market investments in nanoscience and nanoproducts in several areas.Öğe İnstabil çocuk önkol diyafiz kırık cerrahisinde açık ya da kapalı redüksiyonun klinik ve radyolojik sonuçlara etkisi(2015) Sağlam, Necdet; Kurtulmuş, Tuhan; Kıbar, Birkan; Saka, Gürsel; Avcı, Cem Coşkun; Akpınar, FuatAmaç: Stabil olmayan çocuk önkol diyafiz çift kırıklarının cerrahisinde, kırık redüksiyonu her iki kemiği açık, her iki kemiği kapalı ve bir kemiği açık diğeri kapalı yapılan hastaların klinik ve radyolojik sonuçları değerlendirildi. Yöntem : İnstabil önkol 1/4 orta cisim kırığı nedeniyle intramedüller çivileme yöntemiyle tedavi edilen 63 çocuk geriye dönük değerlendirildi. Hastaların 13’üne (Grup A; 3 kız, 10 erkek; ort. yaş 10; dağılım;5 -15) her iki kemiğe açık yerleştirme; 27’sine (Grup B; 4 kız, 23 erkek; ort. yaş 11; dağılım;5 -16) her iki kemiğe kapalı yerleştirme; 23’üne (Grup C; 3 kız, 20 erkek; ort. yaş 11; dağılım;7 -14) bir kemiğe açık diğerine kapalı redüksiyon uygulandı. Gustilo - Anderson sınıflamasına göre 15 hastada tip1, iki hastada tip 2 açık kırık vardı. Yaralanmadan cerrahiye kadar geçen süre grup A’da 4, grup B’de 2, grup C’de 2 gündü. Hastalarda Kirschner teli kullanıldı. Hastaların hepsinde hem radius hem ulna tespit edildi. Ortalama takip süresi grup A’da 38, grup B’de 34, grup C’de 39 aydı. Bulgular: Grup A’da ortalama 47 , grup B’de 45, grup C’de 46 günde kaynama sağlandı. Çivilerin çıkarılma süreleri grup A’da ortalama 53, grup B’de 82, grup C’de 67 gündü. Gruplar arasında yaş, yaralanmadan cerrahiye kadar geçen süre, tel çıkarma ve takip süresi, cinsiyet, taraf, etiyoloji, komplikasyon dağılımlarında istatiksel anlamlı farklılık saptanmadı (p>0.05). Epifiz hasarı, redüksiyon kaybı, refraktür, derin enfeksiyon, kaynamama, nörovasküler yaralanma, Kirschner teli migrasyonu, sinostoz, angular ya da rotasyonel deformite ve ane stezi komplikasyonu gelişmedi. Sonuç : Çocuklarda instabil önkol diyafiz kırıklarının Kirschner teliyle intramedüller tespitinde açık veya kapalı kırık redüksiyonunun fonksiyonel ve radyolojik sonuçlar üzerine etkisi olmadığını istatistiksel olarak gösteril di.Öğe Interlocking intramedullary ulna nails in isolated ulna diaphyseal fractures: a retrospective study(Turkish Assoc Orthopaedics Traumatology, 2013) Saka, Gürsel; Sağlam, Necdet; Kurtulmuş, Tuhan; Özer, Coşkun; Uğurlar, Meriç; Akpınar, FuatObjective: The aim of this study was to evaluate the results of multifunctional intramedullary (IM) ulna nailing following diaphyseal fracture of the ulna. Methods: Adult patients with isolated fractures of the ulna treated with closed or mini-open reduction using the new IM ulna nail between May 2008 and January 2011 and who were followed for a least one year were retrospectively reviewed. Patients with a pathological fracture or nonunion after previous surgeries were excluded. Functional outcome was assessed using the Grace and Eversmann rating system, patient-reported outcomes using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Wrist flexion and extension was evaluated using a goniometer. Results: The 18 patients (13 male, 5 female; mean age: 28 years, range: 18 to 64 years) had a total of 20 isolated ulna fractures (two bilateral). Mean time to fracture union was 13 (range: 10 to 14) weeks. No patient had nonunion, deep infection or radioulnar synostosis. Follow-up time ranged from 12 to 36 months. Grace and Eversmann ratings were excellent in 15 patients, good in 2 and poor in one. Mean DASH score was 8.08 (range: 0 to 17.5) points. Conclusion: The new IM ulna nails may be considered an alternative method for isolated diaphyseal fractures of the ulna. Advantages of this method include its short operative time, insertion by closed and minimal invasive techniques, use of scope only in reduction and locking control, as well as minimal cosmetic defect, small operative scar and early mobilization without additional fixation.Öğe Kirschner wire versus titanium elastic nails in pediatric femoral shaft fractures(Atha Comunicacao & Editora, 2015) Işık, Cengiz; Kurtulmuş, Tuhan; Sağlam, Necdet; Saka, Gürsel; Akpınar, Fuat; Sarman, HakanObjective: To evaluate the effectiveness of intramedullary fixation using the Kirschner-wire (K-wire) compared to the titanium elastic nail (TEN) in pediatric femoral shaft fractures. Methods: A sample of 42 pediatric patients with a mean age of 6.55 +/- 2.42 years (range 4-11 years) presenting femoral shaft fractures underwent intramedullary fixation using the K-wire or TEN. Results: There was no significant difference found between groups, of which 16 (38.1%) patients were treated with K-wire and 26 (61.9%) patients were treated with TEN in terms of union duration. Moreover, none of the patients showed nonunion or a delayed union. Conclusions: The use of adjusted K-wire instead of TEN in the intramedullary fixation of femoral shaft fractures in selected children may be an advantageous surgical option due to the lower cost, easy accessibility and no need for a second surgery for implant removal.Öğe New interlocking intramedullary radius and ulna nails for treating forearm diaphyseal fractures in adults : a retrospective study(Elsevier Sci Ltd, 2014) Saka, Gürsel; Sağlam, Necdet; Kurtulmuş, Tuhan; Avcı, Cem Coşkun; Akpınar, FuatIntroduction: The treatment goal for diaphyseal forearm fractures in adults is to restore axial and rotational stability. The treatment of these fractures with intrmaedullary locked nailing remains sparse. We therefore evaluated IM nails for treating forearm diaphyseal fractures in adults. Methods: We retrospectively reviewed adult patients with isolated unilateral or bilateral fractures of the radius, ulna, or both, who were treated with closed or mini open reduction with a new IM nail between May 2008 and January 2012 and who were followed for a least 1 year. Patients with a Galeazzi fracture, a pathological fracture or patients with nonunion after previous surgeries were excluded. All patients were allowed full range of motion without any external support. Primary outcomes were Grace and Eversmann rating, Disabilities of the Arm, Shoulder and Hand (DASH) scores. Results: The 43 enrolled patients (mean age, 37 years; 32 men) had 59 forearm fractures: 14 isolated radius fractures, 17 isolated ulna fractures (2 bilateral), and 28 fractures of both the radius and ulna. Mean time to fracture union was 13 weeks (range 10-14 weeks) for ulnar fractures and 12 weeks (range 10-13 weeks) for radial fractures. No patient had nonunion, deep infections, or radioulnar synostosis. Followup ranged from 12 to 44 months. Grace and Eversmann ratings were excellent in 38 patients and good in 5. Mean DASH score was 6.5 points (range 0-13.3). Conclusions: Intramedullary nailing of adult forearm diaphyseal fractures appears to be a good alternative to plate osteosynthesis. The advantages are short operative time, minimal invasive techniques, and sufficient stability in all planes that allows early motion without additional fracture support. (C) 2013 Elsevier Ltd. All rights reserved.Öğe Phenol removal from wastewater by surface imprinted bacterial cellulose nanofibres(Taylor & Francis Ltd, 2020) Derazshamshir, Ali; Göktürk, Ilgım; Tamahkar, Emel; Yılmaz, Fatma; Sağlam, Necdet; Denizli, AdilIn this study, we have reported a novel wastewater treatment technique by phenol imprinted bacterial cellulose (BC-MIP) nanofibres with high specificity and adsorption capacity. N-methacryloyl-(L) phenylalanine methyl ester (MAPA) functional monomer was used to create specific binding sites for the template molecule phenol via electrostatic and hydrophobic interactions. BC-MIP nanofibres were synthesized by surface imprinting approach in the presence of different amounts of total monomer (% weight), monomer/template ratio and polymerization time. Then, the nanofibres were characterized by FTIR-ATR, surface area analysis (BET), elemental analysis, scanning electron microscopy (SEM) and contact angle measurements. Adsorption studies were performed with respect to pH, temperature and ionic strength, and the adsorption capacity was calculated by using the spectrophotometer. In order to desorb the adsorbed phenol from BC-MIP nanofibres, 0.1 M NaCl solution was used. Besides, BC-MIP nanofibres were applied to real wastewater samples from Ergene basin in Turkey. The suitable equilibrium isotherm was determined as Langmuir isotherm. To evaluate the selectivity of the BC-MIP nanofibres, similar molecules were utilized as competitor molecules, which were 2-chlorophenol, 4-chlorophenol and 2,4-dichlorophenol. Electrostatic interactions were found to contribute to the generation of specific recognition binding sites. The results have shown that imprinting of phenol was achieved successfully with high adsorption capacity. The phenol removal efficiency was reported up to 97%. BC-MIP nanofibres were used 10 times with a negligible decrease in adsorption capacity. [GRAPHICS] .Öğe Treatment of diaphyseal forearm atrophic nonunions with intramedullary nails and modified Nicoll's technique in adults(Turkish Assoc Orthopaedics Traumatology, 2014) Saka, Gürsel; Sağlam, Necdet; Kurtulmuş, Tuhan; Avcı, Cem Coşkun; Akpınar, FuatObjective: The aim of this study was to evaluate the outcome of tricorticocancellous autologous bone grafting with intramedullary forearm nails in the treatment of radius or ulna aseptic nonunion. Methods: The study included 8 patients (mean age: 39 years; range: 19 to 55 years) who underwent plate-screw osteosynthesis for the treatment of nonunion (6 ulna, 2 radius) following forearm fracture. In all cases, the length of the applied tricortical graft was below 3 cm. Patients were evaluated using the visual analog scale, Grace and Eversmann scale and DASH score. Wrist flexion and extension and postoperative hand and forearm grip strength were assessed. Results: Graft incorporation and union was completed at a mean of 22 (range: 18 to 28) weeks. No patient had nonunion, deep infection or radioulnar synostosis. Follow-up ranged from 18 to 52 months. Radiographic union was achieved in all patients. Mean visual analog scale pain score was 1 (range: 0 to 3). Grace and Eversmann ratings were excellent in 5 and good in 3 patients. Mean DASH score was 10.7 (range: 1.7 to 21.7) points. Uygulanan trikortikal greftin uzunluğu tüm olgularda 3 cm’nin altında idi. Hastalar görsel analog skala, Grace ve Eversmann skalası ve DASH skoru ile değerlendirildiler. Bilek fleksiyon ve ekstansiyonu ile ameliyat sonrası el ve ön kol kavrama kuvvetleri incelendi.Bulgular: Greft inkorporasyonu ve kaynamanın 22 (dağılım: 18-28) haftada tamamlandığı görüldü. Hastaların hiçbirinde kaynamama, derin enfeksiyon veya radyoulnar sinostoz bulgusu yoktu. Takip süresi 18 ile 52 ay arasındaydı. Kaynama tüm hastalarda radyografilerle doğrulandı. Ortalama görsel analog skala ağrı puanı 1 (dağılım: 0-3) olarak belirlendi. Grace ve Eversmann skorları 5 hastada mükemmel, 3 hastada iyi idi. The purpose of this study is evaluating the results of intramedullary (IM) radius nail for the treatment of isolated adult diaphyseal fractures of the radius. Materials and methods: We retrospectively reviewed adults with isolated fractures of the radius, who were treated with closed or mini open reduction with a IM radius nail between May 2008 and November 2011 and who were followed for a least 1 year. Patients with a Galeazzi fracture, a pathological fracture, or patients with nonunion after previous surgeries were excluded. All patients were allowed full range of motion without any external support. Primary outcomes were Grace and Eversmann rating, Disabilities of the Arm, Shoulder and Hand (DASH) scores. Results: Twenty-three enrolled patients (mean age 34 years; 17 men) had 23 isolated radius fractures. Mean time to fracture union was 12 weeks (range 10–13 weeks) for radius fractures. No patient had nonunion, deep infections, or radioulnar synostosis. Follow-up ranged from 12 to 42 months. Grace and Eversmann ratings of 21 patients were excellent or good, and 2 patients were medium. Mean DASH score was 4.2 points (range 0–13.3). Conclusions: Our experience indicates that this new IM radius nail may be considered as an alternative to plate osteosynthesis for fractures of the radius diaphysis in adults. IM nailing of radius fractures provides reliable bony union and excellent postoperative clinical results in adults.
