    Ameloblastomlar çenenin lokal agresif lezyonlarıdır. Anatomik bölge, klinik tablo ve histopatolojik farklılıklara göre farklı alt tipler tanımlanabilir. Bu olguda mandibula gövdesinde enükleasyon ve küretaj ile tedavi edilen unikistik akantamöz ameloblastom sunulmuştur. Unikistik ameloblastom çenenin nadir lezyonlarından olmasına rağmen mandibula gövdesinde görülebilmektedir.

    Acquired toxoplasmosis of the buccal area with extranodular involvement: Report of an unusual case

    Acquired toxoplasmosis is a common parasitic infection in humans. It can be caused by ingestion of infected meat or other food that has been contaminated by the feces of infected cats. Approximately 90% of immunocompetent patients with acquired toxoplasmosis are asymptomatic and undiagnosed; in the other 10%, toxoplasmosis manifests as a nonspecific, self-limited illness that usually does not require treatment. In symptomatic cases, cervical lymphadenopathy is one of the most common clinical findings. We report the case of a 33-year-old woman who experienced unilateral facial swelling secondary to toxoplasmosis. In addition to the atypical location of her disease (i.e., the buccal area), the atypical histopathologic findings in this case (e.g., extranodular involvement) constituted a very unusual presentation of toxoplasmosis.

    Adult-type granulosa cell tumor with splenic metastasis: a rare case

    To the Editor, Granulosa cell tumors are rare malignant tumors defined by Rokitansky in 1855 and consist 2-3% of all ovarian tumors1. Incidence rate is 0.5-1.5/100.000 cases per year2. Characteristically it grows very slowly yet metastasizes years later after the curative tratment 1. The mean age is 50 years but it can be seen at any age after the menopause and patients admit to the clinics with complaints of uterine bleeding, pain and symptoms secondary to pressure2,3.     Association of extent and aggressiveness of inflammation with serum PSA levels and PSA density in asymptomatic patients

    OBJECTIVES The relationship between serum prostate-specific antigen (PSA) levels and histologic prostatic inflammation is controversial. Previous studies were performed using either the intensity or extent of inflammation for grading, with a relatively lower number of specimens. In our study, the inflammation was categorized more profoundly, using both the intensity and the extent of inflammation, to determine the influence of prostatic inflammation on serum PSA levels, percent free PSA (%fPSA), and PSA density (PSAD). METHODS The study included 115 patients who underwent transrectal ultrasound-guided prostate biopsy. To categorize the inflammation, a grading method that included the intensity and extent of inflammation was used. The extent and aggressiveness of inflammation were analyzed. The patients were divided into groups using five grades for the extent and four grades for the aggressiveness of inflammation. The serum PSA Levels, fPSA levels, %fPSA, and PSAD in each group were compared. RESULTS The extent of inflammation grade correlated positively with the serum PSA level (r = 0.423, P <0.001) and PSAD (r = 0.319, P = 0.001). However, a negative correlation was found between the extent of inflammation grade and %fPSA (r = -0.268, P = 0.015). The aggressiveness of inflammation grade correlated positively with the serum PSA level (r = 0.386, P <0.001) and PSAD (r = 0.341, P = 0.001) and negatively with %fPSA (r = -0.289, P = 0.03). CONCLUSIONS If the elevation of serum PSA is thought to be caused by histologic inflammation, it might prevent unnecessary repeated biopsies.

    Bilateral nevus comedonicus of the eyelids associated with bladder cancer and successful treatment with topical tretinoin

    Dear Editor Nevus comedonicus (NC) is a rare epidermalhamartoma of unknown etiology, most com-monly located on the head and neck (1). It ischaracterized by groups of dilated follicular ori-fices resembling comedones, filled with firmhyperkeratotic plugs (1). Although most casesare located unilaterally on the head, neck, andtrunk, the lesions may have a linear or blasch-koid distribution and rarely occur bilaterally(2–8). Histopathologically, the lesions are char-acterized by closely arranged, dilated-invaginated follicular openings containinglamellar keratin and cystic formations. Half ofthe cases are congenital and half develop duringchildhood, usually before the age of 10 years(4–6). Most NC cases are isolated. When NC isassociated with anomalies in the central nervesystem, eye, or skeletal system, and adnexaltumors, it is called NC syndrome (6). We reportlate-onset NC of both eyelids due to its rarity,atypical bilateral localization, and association with bladder cancer.

    A case of colistin-induced fixed drug eruption

    Fiks ilaç erüpsiyonu (FİE) etyolojisinde başta antimikrobiyaller olmak üzere çeşitli ilaçlar rol almaktadır. Klinik görünümü, ilaca bağlı gelişen bir reaksiyon için oldukça tipiktir. Literatürde, kolistine bağlı gelişen fiks ilaç erüpsiyonu olgusu bildirilmemiştir. Acinetobacter pnömonisi nedeniyle kolistin kullanılan 83 yaşındaki bir erkek hastada ortaya çıkan fiks ilaç erüpsiyonu, çok nadir görülmesi açısından sunulmuştur. FİE ayırıcı tanısında kolistinin yer alabileceği akılda tutulmalıdır

    Collision tumour of trichofolliculoma and basal cell carcinoma

    A 52-year-old woman presented with a 2-year history of a reddish nodule on the right nasolabial sulcus. Histopathological examination revealed that the nodule was composed of trichofolliculoma and basal cell carcinoma. There was no transitional zone between the two neoplasms. The diagnosis was made as a collision tumour of trichofolliculoma and basal cell carcinoma.

    A combination of plasma DAO and citrulline levels as a potential marker for acute mesenteric ischemia

    Introduction: There is no valid and reliable diagnostic test for early diagnosis of acute mesenteric ischemia (AMI). The aim of this study was to measure the plasma levels of diamine oxidase (DAO) and citrulline in AMI to gain insight into its early diagnosis. Material and methods: A total of 21 Wistar albino rats were divided into three groups, that is, control group, short-term ischemia group, and prolonged ischemia group. The superior mesenteric artery was occluded for 15 min in the short-term ischemia group and for 12 h in the prolonged ischemia group. Twelve hours later, the experiment was terminated and plasma DAO and citrulline levels were measured. Intestinal tissue was evaluated for the histopathological changes. Results: Compared to the control group, the short-term and prolonged ischemia groups showed significant increases in the plasma levels of DAO, whereas the plasma citrulline levels decreased significantly. Prolonged ischemia caused a larger increase in the plasma DAO levels and a larger decrease in the plasma citrulline levels compared to the short-term ischemia (p =0.011 and p =0.021, respectively). Intestinal damage was shown to develop more in the prolonged ischemia group (p =0.001). Conclusion: In the early period of AMI, the plasma DAO levels increase while citrulline levels decrease, and the extent of these changes depends on the duration of ischemia.

    Comparison of ocular surface side effects of topical travoprost and bimatoprost

    Purpose: To compare the subjective symptoms, conjunctival hyperemia, tearing response and conjunctival cytological changes secondary to topical administration of bimatoprost and travoprost for 6 months. Methods: Newly diagnosed primary open-angle glaucoma patients were randomly prescribed bimatoprost (35 cases) or travoprost (42 cases). Two patients in each group were excluded because they did not appear at their appointments regularly. Thus, 33 and 40 patients completed the study in the bimatoprost and travoprost groups, respectively. Redness, itching, foreign-body sensation, pain and discomfort were assessed by a questionnaire, and patients were examined for conjunctival hyperemia. Schirmer's I and break-up time tests were performed, and impression cytology of conjunctiva was evaluated. Results: Subjective symptoms were similar in both groups. The only subjective symptom that changed significantly was redness. The change in conjunctival hyperemia along the study period correlated with the patient-reported redness in both groups, being highest on day 30. Schirmer's test I and break-up time did not change with time and were similar in both groups. The impression cytology grade increased with time in both groups with the only significant difference between groups on day 90 (higher in the bimatoprost group). Conclusion: We observed conjunctival hyperemia as the most common side effect of bimatoprost and travoprost. Tear film functions were not affected by these drugs while cytological alterations were.     Correlation of proliferating cell nuclear antigen and bcl-2 expression with tumor front grading and metastasis in laryngeal squamous cell carcinoma

    This study was designed to examine the immunohistochemical expression of proliferating cell nuclear antigen (PCNA) and bcl-2 protein in 45 cases with advanced laryngeal squamous cell carcinoma who had undergone total laryngectomy with unilateral modified radical neck dissection, and the relation of this expression to some prognostic factors such as tumor front grading and neck lymph node metastases. Sections were reevaluated for routine histologic grade, tumor front grading and neck lymph node metastases, and were stained with monoclonal antibodies against PCNA and bcl-2. Significant correlation was present between the severity of PCNA expression and incidence of lymph node metastasis (p<0.05). No correlation was found between the severity of PCNA expression and tumor front grading. Bcl-2 expression did not associate with either parameters. In conclusion, PCNA is important in predicting prognosis and no association is present between the bcl-2 protein expression and prognostic factors.

    Diffuse gastric mantle cell lymphoma resembling gastrointestinal stromal tumors

    Do gene expressions differ between gastritis with and without helicobacter pylori?

    Objective: Helicobacter pylori infection is a well-defined risk factor for the development of gastric adenocarcinoma and mucosa-associated lymphoid tissue (MALT) lymphoma. However, mechanisms of carcinogenesis in Helicobacter gastritis need to be elucidated. We aimed in this study to determine the immunohistochemical expressions of some cancer-associated genes, and to show differences between Helicobacter gastritis and Helicobacter-negative gastritis. Material and Methods: Sixty-three endoscopic biopsy samples were selected. The samples comprised of normal gastric mucosa (20 samples), Helicobacter gastritis (22 samples), and Helicobacter-negative gastritis (21 samples). Paraffin sections of samples were processed immunohistochemically with some suppressor genes (Rb, p53), protooncogenes (EGFR, cyclin D1), and a heat shock protein, HSP 105. The gene expressions in both crypt epithelia and lymphoid infiltrate were evaluated separately. Results: In epithelia; expressions of cyclin DI, p53, HSP 105, and EGFR were higher in Helicobacter-negative gastritis than in Helicobacter gastritis (p< 0.0001). Immune expression of HSP 105 in Helicobacter gastritis was lower than in control group (p= 0.008) as well as in Helicobacter-negative gastritis. However, expression of Rb was higher in Helicobacter gastritis than in Helicobacter-negative gastritis (p= 0.034). In lymphoid infiltrate; immune reactions for p53 and cyclin D1 were negative in all samples. Expressions of Rb, EGFR, and HSP 105 in lymphoid infiltrate were similar to expressions in epithelia for all groups. Conclusion: We can suggest that development of gastric cancer has a different pathway in Helicobacter gastritis when compared to the one Helicobacter-negative gastritis. In addition, decrease of heat shock proteins in Helicobacter gastritis may lead to sensitivity of crypt epithelia and lymphoid infiltrate for carcinogenic mutations.

    Does maternal nicotine exposure during gestation increase the injury severity of small intestine in the newborn rats subjected to experimental necrotizing enterocolitis

    Background/Purpose: The aim of this study was to evaluate the effects of maternal nicotine exposure during gestation on injury severity of small intestine in the newborn rats subjected to hypoxia-reoxygenation and cold stress. Methods: A total of 21 Sprague-Dawley pregnant rats were divided into 3 equal groups. The groups were labeled as group 1, control group; group 2, hypoxia-reoxygenation group; and group 3, nicotine-hypoxia-reoxygenation group. The rats of group 3 were exposed to nicotine via subcuticular injection for the last week of gestation (2 mg/kg/d). Newborn rats were collected immediately after birth to prevent suckling of maternal milk (40 rat pups in group 1, 43 rat pups in group 2, and 41 rat pups in group 3). Litters in groups 2 and 3 were stressed twice daily with asphyxia followed by cold (4 degrees C for 10 minutes) stress to induce hypoxic intestinal injury which is relevant to human necrotizing enterocolitis. Breathing 100% CO2 for 10 minutes in a chamber followed by 10-minute 100% O-2 breathing was the asphyxia model repeated twice daily. After hypoxia-reoxygenation and cold stress, newborn rats were returned to their mother's cages. This protocol was repeated for the following 2 days, and the rat pups were decapitated on the third day. Using this protocol of asphyxia and cold stress, all of neonatal rats developed clinical and pathological signs of hypoxia-induced intestinal injury. The entire gastrointestinal tract was removed and examined macroscopically. A 2-cm section of distal ileum from each animal was taken for histopathological and biochemical examinations. Histological changes in ileal architecture were scored and graded from 1 to 5. The remaining intestinal tissues of the animals were used for lipid peroxidation analysis. Results: Typical signs of hypoxia-induced intestinal injury were observed in the 2 experimental groups (groups 2 and 3) macroscopically. There were more grades 3 and 4 injuries in group 3 (P <.05). The malondialdehyde levels were elevated in groups 2 and 3 (P <.001). The malondialdehyde levels of the group 3 were also significantly higher than group 2 (P <.01). Conclusions: Maternal nicotine exposure during gestation results in higher grade histological injury in newborn rats subjected to hypoxia-reoxygenation and cold stress.

    Does serum prostate specific antigen levels correlate with the prostatic inflammation in elderly patients without clinically proven prostate cancer?

    Objectives: To determine the whether histological prostatic inflammation correlates with serum prostate specific antigen (PSA), free PSA (fPSA) and percent of fPSA (%fPSA) levels in elderly patients without clinically proven prostate cancer. Methods: A total of 115 patients without clinically proven prostate cancer with transrectal prostate biopsy were included in this retrospective study. Patients were divided two main groups as patients with and without histologic prostatic inflammation. A grading of the histological prostatic inflammation was performed and patients with prostatic inflammation were divided into three subgroups. The age, prostate volume, serum PSA, fPSA and %fPSA levels were compared between patients with and without prostatic inflammation. Correlation between the parameters and grade of prostatic inflammation was also investigated. Results: Serum PSA and %fPSA levels were significantly higher in men with histologically proven prostatic inflammation (15.47 ± 15.28 ng/mL vs. 11.67 ± 8.12 ng/mL; p = 0.002 and 19.8 ± 0.7 vs. 15.79 ± 0.9; p = 0.01, respectively). The mean serum PSA levels were significantly different among the subgroups (p = 0.02) and prostatic inflammation correlated positively with the PSA levels (r = 0.320, p < 0.001).Conclusions: Our findings suggested that reporting the grade of prostatic inflammation in elderly patients may help avoiding unnecessary repeat biopsies if elevated serum PSA level is the only indication for initial prostate biopsy.
