Öğe Angular measurements of the mandible in adults with temporomandibular joint disorders: a CBCT study(2020) Meyvaci, Seda Sertel; Bulut, Duygu Göller; Koçak, Ayşe Tuğçe Öztürk; Ankarali, HandanObjectives: Temporomandibular disorder (TMD) is a degenerative musculoskeletal disease of unknown etiology, associatedwith morphological and functional deformities. The present study aimed to evaluate the angular parameters of the mandiblein TMD patients with cone-beam computed tomography (CBCT) and to compare with healthy controls. Methods: A total of 107 patients (54 in the TMD group and 53 in the control group) were included in the study. Ten angularmeasurements including right and left sides and 4 different length measurements were evaluated on CBCT images of bothgroups to eliminate individual differences. The differences between the two groups were examined using the significance testor Mann-Whitney U test. Multiple linear regression analysis was used for a detailed examination of the relationship betweenparameters. Results: The upper face width was significantly higher in the TMD group (p=0.004). After correcting for the upper face widthvalue, there was a significant difference between the groups in terms of the right ? angle values (p=0.001). The other differences were not significant (p>0.05). Conclusion: The decrease in the right ? angle in the TMD group can be interpreted as a result of the mechanical effect ofmasticatory muscle hyperactivity on the angular properties of the mandible in these patients.Öğe The Anti-Convulsant Effects of Carvacrol in Penicillin- and Pentylenetetrazole-Induced Rat Models of Epilepsy(Maik Nauka/Interperiodica/Springer, 2024) Eksik, Handan; Ankarali, Seyit; Torun, Ibrahim Ethem; Kilinc, Erkan; Ankarali, HandanEpileptic seizures are caused by abnormal neuronal excitation. It is well established that carvacrol, a monoterpenoid phenol, is able to inhibit the voltage-gated sodium channels and L-type Ca2+ channels and enhance activation of GABA(A) receptors. We therefore hypothesize that carvacrol may prevent epileptic seizures by inhibiting neuronal cation influx and facilitating anion influx. Herein, we investigated possible anti-convulsant effects of carvacrol on penicillin-induced epileptiform activity and pentylenetetrazole (PTZ)-induced seizures in rats. 63 male Wistar rats were assigned to the penicillin- and the pentylenetetrazole-induced groups. Both subgroups received three different doses of carvacrol (25, 75, and 150 mg/kg) intraperitoneally. Seizure stage, onset-times of both myoclonic-jerk and generalized tonic-clonic seizures and the duration of generalized tonic-clonic seizure were evaluated using Racine's scale in PTZ group, while spike frequency and the amplitude of epileptiform discharges were evaluated in the penicillin-induced group. The administration of carvacrol significantly extended the onset time of the first myoclonic jerk (150 mg/kg, p = 0.019) and decreased the number of spike-waves (75 mg/kg, p = 0.033). This study showed that carvacrol has the anti-convulsant effect. However, this effect was observed at a low level. The limited the anti-convulsant effect of carvacrol may be due to its insufficient of acute effect related to the transition of carvacrol to the epileptic focus. More studies are needed to evaluate the effect of carvacrol on chronic epilepsy models and its molecular and pharmacokinetic mechanisms.Öğe Biakromial, Bitrokanterik ve Biiliak Çap Ölçümleri ile Boy Uzunluğu Arasındaki İlişkinin Modellenmesi(2021) Meyvaci, Seda Sertel; Ankarali, HandanAmaç: Bu çalışmanın amacı, Türkiye’de yaşayan Türk popülasyonundaki\rgenç yetişkinlerde biakromial çap (BaC), bitrokanterik\rçap (BtC) ve biiliak çap (BiC) ölçümleri ile boy uzunluğu arasındaki\rilişkiyi her iki cinsiyette en iyi tanımlayan modeli bularak bütünlüğü\rbozulmuş ceset veya iskelet kalıntıları üzerinden yapılan biyolojik profil\rgeliştirme sürecine bu modeli kullanarak katkıda bulunmaktır. Gereç\rve Yöntemler: Çalışmada, yaşları ve boy uzunlukları bilinen sağlıklı 62\rkadın ve 63 erkek olmak üzere toplam 125 yetişkin bireyde dijital kaliper\ryardımıyla BaC, BtC ve BiC değerlendirilmiştir. Çap ölçümleri\rile boy uzunluğu arasındaki doğrusal ilişkiler çoklu doğrusal regresyon\rmodeli ile, doğrusal olmayan ilişkiler ise çokdeğişkenli uyarlanabilir\rregresyon eğrileri ile incelenmiştir. İstatistik anlamlılık düzeyi olarak\rp<0,05 kabul edilmiştir ve hesaplamalarda SPSS 22.0 ve SPM®8 Trial\rprogramları kullanılmıştır. Bulgular: Üç ayrı çap ölçümü yardımıyla,\ryetişkin kadın ve erkeklerde boy uzunluğu ölçümleri ayrı ayrı tahmin\redilmek istendiğinde kadınlarda, sadece BaC ile boy uzunluğu arasında\ranlamlı doğrusal bir ilişki olduğu (p=0,011) ve BaC bir birim\rarttığında boy uzunluğunun da 1,015 cm arttığı belirlenmiştir. Erkeklerde\rise BaC ile boy uzunluğu arasında anlamlı doğrusal bir\rilişki olduğu (p=0,004) ve BaC bir birim arttığında, boy uzunluğunun\rda 1,375 cm arttığı, ayrıca BiC ile boy uzunluğu arasında anlamlı\rdoğrusal bir ilişki olduğu (p=0,004) ve BiC bir birim arttığında boy\ruzunluğunun da 1,186 cm arttığı görülmüştür. Sonuç: Bu çalışmanın\rsonuçları, kadınlarda BaC, erkeklerde ise BaC ve BiC ölçümlerinin\rTürkiye popülasyonunda boy uzunluğu tahmini için bütünlüğü bozulmuş\rcesetlerde veya tamamen iskelet hâline gelmemiş kalıntılarda kullanılabileceğini\rgöstermiştir.Öğe Bitrokanterik Çap Mesafesi Genç Yetişkin Erkeklerin Flamingo Denge Testi Sonuçlarını Etkiler Mi?(2020) Meyvaci, Seda Sertel; Ankarali, HandanBu çalışmada, sağlıklı genç erkeklerde bitrokanterik çap ve Flamingo denge testi performans düzeyi arasındaki ilişkiyi incelemek amaçlandı. Araştırmaya 18-25 yaş aralığında 96 sağlıklı genç erkek alınmıştır. Olguların beden kitle indeksi normal aralıkta 20-24,99 kg/m2 olmak üzere sınırlandırılmıştır. Olguların denge performansı Flamingo Denge Testi ile değerlendirilmiştir. Bitrokanterik çap ölçümü başta olmak üzere ayak parametreleri dijital kaliper kullanarak değerlendirilmiştir. Flamingo denge testi ile bitrokanterik çap arasındaki basit ve kısmi korelasyonlar Pearson korelasyon analizi ile değerlendirildi. Bu çalışmadan elde ettiğimiz sonuçlar sağlıklı genç erkeklerde bitrokanterik çap ve Flamingo denge testi performans düzeyi arasında, ayak parametreleri ve Beden Kitle İndeksi değerleri kontrol altına alınarak ve alınmayarak istatistiksel olarak incelendi. Flamingo denge testi ile bitrokanterik çap arasındaki ilişki her iki değerlendirmede de istatistiksel olarak anlamlı bir ilişki olmadığını göstermiştir (p>0,05). Çalışmamızın sağlıklı erkek genç yetişkinlerde trokanterik çap parametresi ve Flamingo denge performansı biyomekaniği arasındaki ilişkinin anlamlı bulunmaması ile bu sonuçların ışığında ileride yapılacak klinik araştırmaların planlanmasında katkı sağlayarak yararlı olabileceği kanısındayız.Öğe Dental age estimation with special emphasis on age limits of 12/15 and 18 years: Detailed analysis according to governing law(Marmara Univ, Inst Health Sciences, 2023) Sinanoglu, Alper; Kunuk, Feyza Akalin; Bayrak, Seval; Ankarali, HandanObjective: The aim of this study is to investigate the potential of Demirjian method in estimation of age groups with limits of 12, 15, and 18 years, according to ages of legal responsibilities. Methods: The panoramic radiographs of the study population aged between 6 to 22 were assessed for third molar (3M) mineralization with Demirjian method with four subgroups. Group 1 consisted of individuals aged between 6 to and 12 years of age, Group 2 were aged between 12 to 15 (Group 2, >= 12, <15), Group 3 were aged between 15-18 years (Group 3, >= 15, <18) and Group 4 were aged over-18s (Group 4, >= 18). Chronological age, developmental stages of 3Ms, differences between bilateral 3M stages and their relations between age groups were evaluated. Results: Regarding all 3M mineralization evaluations, stages 0 and A for Group 1; stages C and D for Group 2; stages E and F for Group 3; stage H for Group 4 was found to be high in percent (p<.05). Regarding the bilateral evaluations, the most variations were in readings of stages B, C, E and G as two stages below and one stage above the concordance. Conclusion: Use of Demirjian method facilitated discrimination of specific age groups with 12, 15, and 18 age thresholds in a sample of a young Turkish subpopulation from the Northwest Anatolia. Regarding the staging assessment in Demirjian method, independent evaluation of each side must be considered for bilateral 3Ms.Öğe Effect of foot anthropometric measurements on postural stability(2020) Meyvaci, Seda Sertel; Meyvaci, Turgut; Kosif, Rengin; Diramali, Murat; Ankarali, HandanAim: To examine the effect of foot anthropometric measurements and body sizes of young maleadults with normal posture on balance.Methods: In this study, the effect of body size and foot anthropometric measurements of 112 youngmale adults with normal posture on balance was investigated. The foot and body parameters of thecases were measured. The static and the dynamic balance tests were evaluated according to thedominant foot in each case. The parameters that affected balance were determined and the variableswere taken to the model. In addition, the significance levels that defined the effects of the propertiesexamined in relation with the balance were also calculated.Results: When the findings were evaluated, it was determined that the effect of the foot parametersother than the foot length, and the effect of 15 body parameters other than the biiliac diameter,trochanteric height, and right upper extremity length on balance performance was significant. Thebalance test performance was predicted with success ranging from 7.8% to 43% with the parametersincluded in the model.Conclusion: In this study, the fact that the relation between the foot anthropometric and bodydimensions and functional balance performances of young male adults was found to be significantshows that this relation must be considered in the creation of a normative database on balance, and inclinical studies that will be conducted on the subject.Öğe Gender Estimation from Angular Parameters of Mandible in Turkish Adults(2021) Meyvaci, Seda Sertel; Bulut, Duygu Göller; Koçak, Ayşe Tuğçe Öztürk; Ankarali, HandanAim: In this study, it was aimed to evaluate the effect of angular parameters, which are a part of mandibular morphometry, on gender assessment with the help of cone beam computed tomography (CBCT). Material and Methods: A total of 197 healthy adults, 100 females and 97 males, aged between 18- 68 years were included in our study. Ten angular parameters of the mandible were evaluated using CBCT. Independent samples t-test was used to compare the gender for each of the age and angle measurements. In addition, the success of the angles in gender diagnosis was examined by a stepwise linear canonical differential analysis. Statistical significance level was accepted as p<0.05. Results: When we compare mandible angular parameters and age measurements between gender groups, left Go angle (p=0.026), mentomandibular angle (p=0.007), right ? angle (p=0.002), right ? angle (p=0.001), left ? angle (p=0.009) and age (p=0.014) values differed significantly, and the remaining 5 angles were similar (p>0.05). In order to eliminate the effect of age difference, a total of 11 variables including age were comprised in the model, and discriminant analysis was performed with the stepwise variable elimination method to select only the variables that could make significant gender discrimination. As a result of variable elimination, we found that the overall correct classification success of the discriminant function, which includes a total of 4 angles, sexually dimorphic right ? angle, left Go angle, right ? angle, and left ? angle, was 71.5%. Conclusion: This study showed that among the angular parameters of the mandible, the left Go angle being the highest, the right ? angle, right ? angle and left ? angles have sexual dimorphic properties, and gender estimation can be made with 71.5% overall accuracy with the discriminant function containing these parameters.Öğe Identifying risk factors for blood culture negative infective endocarditis: An international ID-IRI study(Elsevier Sci Ltd, 2024) Filiz, Mine; Erdem, Hakan; Ankarali, Handan; Puca, Edmond; Ruch, Yvon; Santos, Lurdes; Fasciana, TeresaBackground: Blood culture-negative endocarditis (BCNE) is a diagnostic challenge, therefore our objective was to pinpoint high-risk cohorts for BCNE. Methods: The study included adult patients with definite endocarditis. Data were collected via the Infectious Diseases International Research Initiative (ID-IRI). The study analysing one of the largest case series ever reported was conducted across 41 centers in 13 countries. We analysed the database to determine the predictors of BCNE using univariate and logistic regression analyses. Results: Blood cultures were negative in 101 (11.65 %) of 867 patients. We disclosed that as patients age, the likelihood of a negative blood culture significantly decreases (OR 0.975, 95 % CI 0.963-0.987, p < 0.001). Additionally, factors such as rheumatic heart disease (OR 2.036, 95 % CI 0.970-4.276, p = 0.049), aortic stenosis (OR 3.066, 95 % CI 1.564-6.010, p = 0.001), mitral regurgitation (OR 1.693, 95 % CI 1.012-2.833, p = 0.045), and prosthetic valves (OR 2.539, 95 % CI 1.599-4.031, p < 0.001) are associated with higher likelihoods of negative blood cultures. Our model can predict whether a patient falls into the culture-negative or culture-positive groups with a threshold of 0.104 (AUC +/- SE = 0.707 +/- 0.027). The final model demonstrates a sensitivity of 70.3 % and a specificity of 57.0 %. Conclusion: Caution should be exercised when diagnosing endocarditis in patients with concurrent cardiac disorders, particularly in younger cases.Öğe The Importance of the Trigeminal Cardiac Reflex in Rhinoplasty Surgery(Lippincott Williams & Wilkins, 2015) Ozcelik, Derya; Toplu, Gaye; Turkseven, Arzu; Sezen, Gulbin; Ankarali, HandanBackground Trigeminocardiac reflex (TCR) consists of bradycardia or asystole along with hypotension and apnea coinciding with stimulation of the trigeminal nerve. During rhinoplasty procedures, we noticed that local anesthetic solution (LAS) application to the columellar area results in bradycardia. We planned to conduct a randomized prospective study on 47 patients undergoing rhinoplasty to demonstrate the characteristics of TCR arising from the columella. Method Local anesthetic solution containing 2% prilocaine with 1:80,000 adrenaline was applied under standard general anesthesia protocol. In group 1 (study group, n = 24), 2 mL of LAS was applied to the columella. In group 2 (control group, n = 23), 2 mL of LAS was applied to the nasal dorsum. In group 3 (control group, n = 20), after LAS was applied to nasal dorsum in group 2, we waited for 10 minutes. Then, 2 mL of LAS was applied to the columella. Here, recordings were taken for the columella. Heart rate (HR) and blood pressure (BP) were recorded just before needle insertion (baseline level), at the time of needle insertion (NIT) to the columella or dorsum, and after the 1st, 5th, 10th, 30th, and 60th seconds. Results Transient bradycardia (>= 20% drop in HR) was observed in 33% of the patients in group 1. Decrease in HR compared to the baseline level in group 1 was significantly greater than that of groups 2 and 3 at all times (P <= 0.05). Systolic BP in NIT and in 60th second in group 1, only in NIT in group 2 was significantly lower than that of baseline levels (P <= 0.05). Conclusions We concluded that stimulation of a sensory branch of the trigeminal nerve in the columellar area leads to TCR under general anesthesia by eliciting clinical hypotension with a drop in systolic BP and in HR of more than 20% compared to the baseline level. Knowing the existence of a certain TCR area will be helpful to the surgeon and anesthesiologist to exercise extra vigilance and to make continuous and meticulous monitoring of the electrocardiogram, HR, and BP during which the TCR may be precipitated such as local anesthetic infiltration to the columellar area in rhinoseptoplasty operations.Öğe Obez Bireylerde Vücut Kompozisyonu Bileşenlerinin Modellenmesi(2021) Meyvaci, Seda Sertel; Ankarali, HandanAmaç: Bu çalışmada obez bireylerin vücut kompozisyon bileşenleri modellenerek vücut analizi cihazı ile ağırlığı ölçülemeyen morbid obezlerin vücut kompozisyon bileşenlerinin tahmininin yapılması amaçlandı. Gereç ve Yöntemler: Beslenme ve Diyet Polikliniği’ne başvuran 160’ı kadın (%75,5), 52’si erkek (%24,5) olmak üzere toplam 212 yetişkin obez bireyin vücut kompozisyonu parametreleri retrospektif olarak incelendi. Yağ miktarı (YM), yağsız doku miktarı (YDM), sıvı miktarı (SM) ve yağ yüzdesi (YY) gibi vücut kompozisyon ölçümleri segmental vücut analizi cihazı ile değerlendirildi. Ayrıca bireylerin yaşcinsiyet gibi demografik özellikleri ile boy uzunluğu ve beden kütle indeksi verileri hastane arşivinden alınarak incelendi. İstatistiksel analizlerde parametreler için ortalama, standart sapma ve yüzde değerleri kullanıldı. Çalışmada yer alan yaş-cinsiyet verileri ile boy uzunluğu, vücut ağırlığı, YM, YDM, SM ve YY ölçümleri arasındaki ilişkilerden parametrik olmayan regresyon modeli kullanılarak 4 farklı model geliştirildi. Bulgular: Geliştirilen modellerde yapılacak tahminlerdeki isabet derecesi en yüksek %82,3 ile SM’de ve en düşük de %50 olarak YY’de bulundu. Vücut ağırlığı, boy uzunluğu ve cinsiyetin vücut kompoziyonu bileşenleri üzerine etkisinin olduğu, yaşın ise bu parametreler üzerinde herhangi bir etki göstermediği saptandı. Sonuç: Elde edilen sonuçlarımıza göre geliştirilen modellerle ağırlığı vücut analizi cihazı ile ölçülemeyen morbid obezlerde vücut kompozisyonu parametrelerinin tahmin edilebileceğini ve bu tahminlerin klinikte kullanılabileceği sonucuna varılmıştır. Çalışmamızın bu yönüyle literatüre katkı sağlayacağı düşünülmektedir.Öğe Protective effects of long-term probiotic mixture supplementation against pentylenetetrazole-induced seizures, inflammation and oxidative stress in rats(Elsevier Science Inc, 2021) Kilinc, Erkan; Ankarali, Seyit; Ayhan, Didem; Ankarali, Handan; Torun, Ibrahim Ethem; Cetinkaya, AyhanEmerging evidence indicates that dysbiosis of gut microbiota plays an important role in epilepsy, although the underlying mechanisms remain unclear due to the complex nature of both microbial composition and pathophysiology of epilepsy. We investigated effects of long-term probiotics supplementation on epileptic seizures, and inflammatory and oxidant/antioxidant biomarkers in a pentylenetetrazole(PTZ)-induced seizure model in rats. Male Wistar weaner-rats were divided into four groups. The first two groups received 1 ml/day saline solution, while the other groups received 0.05 mg/1ml/day vehicle or 109cfu/1ml/day probiotic-mixture, respectively, for 60 days by gavage. Seizure was induced by a single convulsive dose of PTZ. Seizures were evaluated using Racine's scale. Concentrations of pro-inflammatory cytokines in plasma and brain tissue were determined using ELISA, while oxidant/antioxidant biomarkers were measured using an automated-colorimetric method. Probiotics supplementation exhibited anticonvulsant effects against PTZ-induced seizures by retarding onset-times of both myoclonic-jerk and generalized tonic-clonic seizure, and by shortening duration of generalized tonic-clonic seizure. Additionally, it alleviated PTZ-induced increases in levels of pro-inflammatory cytokines IL-1 beta, IL-6, and IL-17A, but not of IFN gamma, in plasma and brain tissue. Moreover, it restored PTZinduced fluctuations in levels of oxidants TOS and disulfide, and of antioxidants native thiol and total thiol. Our findings suggest that long-term probiotics supplementation exhibits protective effects against epileptic seizures, and alleviates (neuro)inflammation and oxidative stress related to pathophysiology of epilepsy. A probiotic-rich diet provided from childhood may provide prophylaxis against epileptic seizures, especially in susceptible individuals, as the neonate diet represents a fundamental extrinsic factor in establishing gut microbiota. All rights reserved.Öğe Survival in rhino-orbito-cerebral mucormycosis: An international, multicenter ID-IRI study(Elsevier B.V., 2022) Cag, Yasemin; Erdem, Hakan; Gunduz, Mehmet; Komur, Suheyla; Ankarali, Handan; Ural, Serap; Tasbakan, MeltemBackground: Mucormycosis is an emerging aggressive mold infection. This study aimed to assess the outcome of hospitalized adults with rhino-orbito-cerebral mucormycosis (ROCM). The secondary objective was to identify prognostic factors in this setting. Methods: This study was an international, retrospective, multicenter study. Patients’ data were collected from 29 referral centers in 6 countries. All qualified as “proven cases” according to the EORTC/MSGERC criteria. Results: We included 74 consecutive adult patients hospitalized with ROCM. Rhino-orbito-cerebral type infection was the most common presentation (n = 43; 58.1%) followed by rhino-orbital type (n = 31; 41.9%). Twenty (27%) had acquired nosocomial bacterial infections. A total of 59 (79.7%) patients (16 in combination) received appropriate antifungal treatment with high-doses of liposomal amphotericin B. Fifty-six patients (75.7%) underwent curative surgery. Thirty-five (47.3%) required intensive care unit admission (27; 36.5% under mechanical ventilation). Hospital survival was 56.8%, being reduced to 7.4% in patients with invasive mechanical ventilation. A multivariate binary backward logistic regression model identified confusion at admission (OR 11.48), overlapping hospital-acquired infection (OR 10.27), use of antifungal treatment before diagnosis (OR 10.20), no surgical debridement (OR 5.92), and the absence of prior sinusitis (OR 6.32) were independently associated with increased risk for death. Conclusion: Today, ROCM still has high mortality rate. Improving source control, rational therpy, and preventing nosocomial infections may improve survival in this severe infection. © 2022
