Öğe Association of toll-like receptors 2, 3, and 4 genes polymorphisms with periapical pathosis risk(Medicina Oral S L, 2016) Özan, Ülkü; Ocak, Zeynep; Özan, Fatih; Oktay, Elif-Aybala; Toptaş, Orçun; Şahman, Halil; Yıkılgan, İhsan; Oruçoğlu, HasanBackground: The aim of this study was to investigate the role of gene variations of Toll-like receptors (TLR) 2, 3, and 4 on genetic susceptibility to periapical pathosis. Material and Methods: One hundred patients were included in the study and divided into two groups as follows; Control Group (n=50) that have root canal treatment and no periapical lesion, Patient Group (n=50) that have root canal treatment and periapical lesion. TLR2 Arg753Gln, TLR3 (c.1377C/T) and TLR4 Asp299Gly and Thr399Ile polymorphisms were genotyped by using PCR-RFLP. Genotypical analysis of control and patient groups were investigated to disclose whether there is any association between periapical lesions and gene variations. Results: There are no significant statistical differences between control and patient groups according to TLR 2 and 4 gene sequence. On the contrary, CC allele detected 74% for TLR 3 in patient group, and this difference was found to be statistically significant (p < 0.005). Conclusions: According to these results, it can be suggested that patients with Toll-like receptor 3 gene polymorphisms could be susceptible to periapical pathosis.Öğe Bilateral glandular odontogenic cyst of mandible: A rare occurrence(Springer India, 2015) Akkaş, İsmail; Toptaş, Orçun; Özan, Fatih; Yılmaz, FahriGlandular odontogenic cysts (GOCs) of the jaw are rare with well-defined limits radiologically, unusual histopathological features and a high recurrence rate. The radiographic appearance of GOCs vary and are not pathognomonic. Definitive diagnosis of the GOC is established only by histopathological examinations. Histologically, GOC is characterized by a thin nonkeratinized squamous epithelial lining, with papillary projections, nodular thickenings, mucous (goblet) cells with intraepithelial mucous pools and intraepithelial glandular, microcystic or duct-like structures. We present an unusual case of a bilateral GOC in the mandible. This case report is also the first documented case of bilateral GOC in the mandible.Öğe Effect of Royal Jelly on new bone formation in rapid maxillary expansion in rats(Medicina Oral S L, 2015) Özan, Fatih; Çörekçi, Bayram; Toptaş, Orçun; Halıcıoğlu, Koray; Irgın, Celal; Yılmaz, Fahri; Hezenci, YasinBackground: The aim of this study was to evaluate the effects of long and short term systemic usage of royal jelly on bone formation in the expanded maxillary suture in a rat model. Material and Methods: Twenty eight Wistar albino rats were randomly divided into 4 equal groups: Control (C); Only Expansion (OE), Royal Jelly (RJ) group, Royal Jelly was given to rats by oral gavage only during the expansion and retention period; Royal Jelly plus Nursery (RJN) group, Royal Jelly was given to rats by oral gavage during their nursery phase of 40 days and during the retention period. After the 5 day expansion period was completed, the rats underwent 12 days of mechanical retention. All rats were sacrificed in same time. Histological examination was performed to determine the number of osteoclasts, number of osteoblasts, number of capillaries, inflammatory cell infiltration, and new bone formation. Results: New bone formation, number of osteoclasts, number of osteoblasts, and the number of capillaries in the expanded maxillary sutures were higher in the RJ and RJN groups than in the other groups. Statistical analysis also demonstrated that new bone formation and the number of osteoblasts was also highest in the RJN group. Conclusions: The systemic administration of Royal Jelly in conjunction with rapid maxillary expansion may increase the quality of regenerated bone.Öğe Effect of St John's Wort on bone formation in the orthopaedically expanded premaxillary suture in rats : a histological study(Oxford Univ Press, 2015) Halıcıoğlu, Koray; Çörekçi, Bayram; Akkaş, İsmail; Irgın, Celal; Özan, Fatih; Yılmaz, Fahri; Türker, ArzuBackground/Objective: The aim of this study was to investigate the effect of systemic St John's wort (Hypericum perforatum) on bone formation in the expanded premaxillary suture in rats. Materials/Methods: A total of 28 rats were randomly divided into four groups of equal numbers: control (C); only expansion (OE); St John's wort extract given only during the expansion and retention period (a total of 17 days; SJW group); and St John's wort extract given during the nursery phase before expansion (a period of 40 days), and during the expansion and retention periods (a total of 57 days; N + SJW group). After the 5 day expansion period was completed, the rats in the OE, SJW, and N + SJW groups underwent 12 days of mechanical retention, following which they were killed, and their premaxilla dissected and fixed. Histological examination was performed to determine the number of osteoclasts and capillaries, as well as the number of osteoblasts, inflammatory cell infiltration, and the amount of new bone formation. Results: Statistical analysis showed that the number of osteoclasts and capillaries, and the inflammatory cell infiltration, as well as new bone formation, were higher in the SJW and N + SJW groups than in the other groups. However, statistical analysis demonstrated that among these two groups, all parameters, with the exception of the number of capillaries, were higher in the N + SJW group than the SJW group. Conclusions/Implications: Although more effective in long-term usage, systemic use of St John's wort hastens new bone regeneration at the premaxillary suture and may help prevent relapse after expansion.Öğe Effects of energy drinks on serum VEGF and bFGF levels during wound healing(Carbone Editore, 2015) Toptaş, Orçun; Tek, Mustafa; Akkaş, İsmail; Kazancıoğlu, Hakkı Oğuz; Tosun, Mehmet; Özan, FatihIntroduction: Energy drinks consist of many substances like caffeine, taurine, sucrose/glucose, vitamins and some herbal supplements like ginseng, gingko biloba and guarana and are used by millions of people world-wide. The study aims to investigate the effect of energy drinks on blood vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) levels. Material and method: Thirty male adult Wistar albino rats were randomly assigned to two groups, an experimental group (EG) and the control group (CG). The Red Bull (3.57 mL/kg/day equivalent to a person weighing 70 kg consuming 250 mL per day) was given by oral gavage to the EG and the same volume of water was given by oral gavage to CG. The full thickness 5 cm diameter skin incisions were performed on the back of the rats. The blood samples were taken from the rats (n=5 in each group) on the 3rd, 7th and 14th days of the experiment. The biochemical blood analysis was performed at the end of the experiment. Results: The 3rd day, mean VEGF values of EG and CO were statistically different. Similarly, on the 14th day, mean bFGF values of EG and CG were statistically different. On the 7th day, there were no statistically differences between mean VEGF and bFGF values of EG and CG. Discussion: The ingredients of energy drinks may affect the wound healing positively or negatively with many mechanisms described in literature. Our results confirmed that energy drinks may accelerate angiogenesis in early and late stages of the proliferation phase of wound healing. Further investigations may reveal more reliable results.Öğe Effects of the topical hemostatic agent Ankaferd Blood Stopper on the incidence of alveolar osteitis after surgical removal of an impacted mandibular third molar(Wolters Kluwer Medknow Publications, 2014) Tek, Mustafa; Akkaş, İsmail; Toptaş, Orçun; Özan, Fatih; Şener, İsmailBackground: Alveolar osteitis (AO) is a commonly seen post-operative complication during the wound-healing period after permanent tooth extraction or surgical removal of impacted third molar teeth. Objectives: The aim of this clinical study was to evaluate the effects of administration of the topical hemostatic agent Ankaferd Blood Stopper (ABS) into the socket on AO formation after impacted mandibular third molar extraction. Patients and Methods: Bilaterally, 100 half-impacted mandibular third molars were extracted in 50 patients. Then, 1.0 mL ABS was administered to achieve hemostasis in one half of the sockets and as a control, the other half was irrigated with 1.0 mL physiological serum after surgery. Results: There was no statistically significant difference in terms of AO formation (P > 0.05) between the extraction sites. However, the postoperative pain in ABS administration sites was higher than in the other sites for the first 2 days after surgery (P < 0.05). Conclusions: The results showed that ABS administration did not increase the incidence of AO formation. Thus, ABS can be used safely for hemostasis after impacted mandibular third molar surgery.Öğe Erişkin Türk toplumunda maksimum ağız açıklığı miktarı ve temporomandibular eklem bozuklukları ile ilişkisinin araştırılması(2013) Ezirganlı, Şeref; Kara, Muhammed İsa; Küçük, Dervişhan; Özan, Fatih; Polat, SerkanAmaç: Bu çalışmanın amacı, erişkin Türk toplumunda maksimum ağız açıklığını tespit etmek ve bu sonucun varsa temporomandibular eklem bozukluğu ile olan ilişkisini tespit etmekti. Gereç ve Yöntem: Çalışmamızda maksimum ağız açıklığı 16–72 yaş aralığında 551’i erkek, 504’ü kadın olan 1055 erişkinden yapılan ölçümler sonucu elde edilmiştir. Yaş, cinsiyet, palpasyonda temporomandi- bular eklem ağrısı, palpasyonda çiğneme ve boyun kaslarında hassasiyet, kapanış tipi, temporomandibular eklemde ses varlığı ve çene ekleminde sorun nedeniyle diş hekimi ya da doktora gitme hikayesi kaydedildi. Bulgular: Maksimum ağız açıklığı erkeklerde 50,38 mm kadınlarda ise 46,35 mm olarak tespit edildi. 32 hastada açık kapanış, 594 hastada normal kapanış ve 429 hastada derin kapanış vardı ve hastaların ortalama ağız açıklıkları sırasıyla 52,53 mm, 48,62 mm ve 47,09 mm olarak kaydedildi. Sonuç: Ağız açıklığı ile kapanış tipi, temporoma dibular eklemde ağrı, çiğneme ve boyun kaslarında ağrı olması arasında ilişki bulundu. Bununla beraber çene ekleminden ses gelmesinin ağız açıklığı üzerine belirgin bir etkisinin olmadığı tespit edildi. Palpasyonda temporomandibular eklem ağrısı olan ve çene eklem şikayeti ile hekimine başvuran gruplarda ağız açıklığı miktarında belirgin bir faklılık olduğu tespit edildi.Öğe Influence of the lower third molar on mandibular angle fractures(2013) Akkaş, Ismail; Tozo?lu, Sinan; Özan, FatihThe purpose of this study was to estimate the relative risk of mandibular angle fractures among people with a lower third molar compared with those without a lower third molar. For this purpose, database research was performed on a whole PUBMED/MEDLINE by using key words "mandibular third molars" and "angle fractures". The data obtained from 9 articles that had a similar statistical analysis; and that included mandibular angle fractures and other mandibular fractures as a control group were evaluated statistically. In these 9 retrospective cohort studies, the sum of cases was 6466 and the sum of mandibular angle fractures was 1769. The analysis of articles shows a relative risk for a mandibula to fracture, comparing patients with and without third molars, ranging from 1,2 to 3.6. The estimated relative risk across the 9 studies was 2.14 (95% CI 1.92-2.39). The results obtained in this study suggest that the presence of lower third molar may double risk of an angle fracture of the mandible.Öğe Propolis accelerates the consolidation phase in distraction osteogenesis(Lippincott Williams & Wilkins, 2014) Bereket, Cihan; Özan, Fatih; Şener, İsmail; Tek, Mustafa; Altunkaynak, Berrin ZuhalWe evaluated the effect of propolis on new bone formation after distraction osteogenesis (DO). This study examined 3 groups: control group, P100, and P200. Rabbits underwent DO of the left mandible after an osteotomy between the first molar and the mental foramen. Bone mineral content and bone mineral density were evaluated using dual-energy x-ray absorption 1 and 4 weeks after the procedure. The volume of connective tissue and new bone and the number of capillaries were measured using stereologic analysis after the subjects were killed. Dual-energy x-ray absorption showed that the bone mineral content and bone mineral density were higher in the groups treated with propolis by week 4, and these parameters were higher in the P200 group. Stereologic analysis showed no significant differences in connective tissue volume and number of capillaries among the groups. New bone volume was lowest in the P200 group. We concluded that propolis accelerates bone formation and may shorten the consolidation phase with DO.Öğe Segmental alveolar osteotomy by palatal approach to correct excessive angulated dental implants in anterior and posterior maxilla(Premchand Shantidevi Research Foundation, 2015) Akkaş, İsmail; Toptaş, Orçun; Akpınar, Yusuf Ziya; Özan, FatihDental implants have been used for a long time to achieve better prosthetic and health conditions in the mouth. With the increase in their usage, more complications have occurred, and methods of solving these problems have been developed. One complication is insertion of the implant in the wrong direction. The aim of this case report is to describe an osteotomy technique to reposition a malpositioned dental implant. A female patient, aged 38 years, and a male patient aged 48 years, were referred complaining of the malpositioned osseointegrated implants, which had been placed in the region of the left maxillary first premolar and molar tooth, and in the region of the left maxillary lateral incisor. Due to severe buccal positioning of the implant fixtures, acceptable prosthetic treatment was not possible. Alveolar osteotomy procedure was used to reposition the implants. Satisfactory results were obtained by osteotomy for 18-month of follow up. We conclude that inadequately axially inclined implants can be successfully treated by alveolar osteotomy. The preservation of marginal gingiva permits obtaining better prosthetic results. To avoid the recession of attached gingiva, palatal approach technique may help the clinicians.
